AFFIDAVIT
I, REMEDIOS V. TUMAMAK, of legal age, Filipino, widow and a resident of Brgy. Suba,
Villaba, Leyte, after having been duly sworn to in accordance with law, do hereby depose and
state that:

1. I am a Roman Catholic having been baptized by virtue of the Sacrament of Holy

Baptism of the Holy Child Parish, Villaba, Leyte, Philippines;

2. I was born on August 27, 1960 in Crossing, Villaba, Leyte to Spouses Diogracio
Montesclaros Villorejo and Asuncion Veral Villorejo;

3. On July 5, 2017, I went to the Office of the Holy Child Parish of Villaba, Leyte
supposedly to get my Baptismal Certificate but no record of my baptism was
found;

4. I hereby testify and attest to the truth that I am a baptized Roman Catholic having
been baptized at the Holy Child Parish, Villaba, Leyte, Philippines, with godparent
Anatolia Talle (deceased);

5. I am willingly and voluntarily executing this Affidavit to attest to the truth of the
foregoing facts and for the purpose of getting a Baptismal Certificate and/or any
Certification with the Office of the Holy Child Parish.

6. This affidavit is also executed for any and whatever legal purpose that this may
serve.
