CONSENT FORM TEMPLATE

The template overleaf should be used when compiling a consent form to be used in your
study. Please follow the layout of the template and use the instructions contained within
to inform your consent form. Please edit the form to reflect your own study and remove
the instruction that we have provided.

Please note it is important to use a font recommended by the British Dyslexia


Association:

Use a plain, evenly spaced sans serif font such as Arial and Comic Sans.
Alternatives include Verdana, Tahoma, Century Gothic, and Trebuchet.
Font size should be 12-14 point. Some dyslexic readers may request a larger
font.

For some studies a fuller, itemised or hierarchical consent form may be needed to cover
important elements of the study, especially if some elements are optional for the
participant. These may include:

additional invasive tests or samples required for study purposes only;


confirmation that the study has been explained appropriately and is
understood by the participant;
consent to use of audio/video-taping, with possible use of verbatim
quotation or use of photographs; or footage being shown to named
individuals;
transfer of data/samples to countries with less data protection;
agreement to receive individual feedback from testing;
agreement of others to access data;
consent to contact participants GP;
agreement to use anonymous data for other purposes (be specific);
notice that the researcher may need to report disclosed sensitive information
such as; illegal activity, malpractice, etc;

Guidelines for Participant Debriefing

Debriefing is an important element of research and is an opportunity, once data


has been collected, for an investigator to provide participants with any necessary
information to complete their understanding of the nature of the research.

A debrief form should include the following:

Title of the study


Thank the participant for taking part in the study
Provide a description on the nature of the investigation. Ensure that the
language used to describe the study is appropriate to the participant
population i.e., a psychology student will have a much greater
understanding of terminology or jargon than a member of the public. This
should include the aim of the research, details of the tasks participants
were asked to undertake, what each task measured and the researcher
hypothesis (if appropriate).
Provide as assurance that data will be held confidentially or anonymously as
appropriate and that participants have the right to withdraw their data
without explanation and retrospectively.
Provide contact details of the researcher

Note: researchers are advised, for their own personal safety, not to use personal
postal addresses, telephone numbers, or e-mail addresses

Debriefing Form

Thank you for participating as a research participant in the present study


concerning project cost estimates in construction projects. The study looks to
investigate the accuracy and importance of conceptual cost estimates.

This study will look at different cost estimate models and how they affect project
development and the quality of a project once it starts.

Again, we thank you for your participation in this study. If you know of any friends
or acquaintances that are eligible to participate in this study, we request that you
not discuss it with them until after they have had the opportunity to participate.

If you have any questions regarding this study, please feel free to ask the
researcher at this time.
