B. ACTIVITIES
The edifice is located at Bonifacio Street in Barangay Poblacion Oeste, Dagupan City
and completed last May, 2016. Supplementary materials from the Central Office have
not yet arrived though a Letter of Request has been sent to the Office of the Secretary
last year to follow-up the library resources to be provided by the Central Office as
stated in the Memorandum of Agreement.
The Division Library continues to build its collection thru book donations from private
individuals and NGOs. For the month of January, the library acquired one (1) book
from DepEd-Disaster Risk Reduction Management Service DRRMS entitled What
Happens in Disasters! A Compilation of Real Accounts from Survivors of Different
Natural Disasters thru Mrs. Marjorie S. Rodriguez, PDO II, DRMM.

A. ACTIVITIES
Monitored the first Simultaneous Earthquake Drill for this year at T. Ayson E.
School of Barangay Calmay, Dagupan City on March 31, 2017.
AMAZING MOTORBOAT RIDE TO ASSESS THE CAPACITY LEVEL OF AN ISLAND SCHOOLS
IN TIME OF DISASTERS. On our way to our respective assigned island school in Dagupan
City to monitor its earthquake preparedness.
DUCK, COVER AND HOLD. Teachers and pupils were actively participated
during the conduct of the first nationwide simultaneous earthquake drill at

Submitted Personal Data Sheet (PDS) using the new form and application of
PAG-IBIG Member I
Prepared Technical Assistance (T.A)/Monitoring Report.
Submitted Periodic Report.
Assisted during NDEP Coordinators Meeting held at Teachers Hall on April 3,
2017.
Submitted an Individual Performance Commitment and Review Form (IPCRF)
for the period of January to December 2016 and reviewed by Dr. Venus Maria
SM. Estonilo, Chief Education Supervisor, CID on April 10, 2017.
Attended an orientation on Work and Financial Plan Activities given by Dr.
Venus Maria SM. Estonilo, Chief Education Supervisor, CID on April 17, 2017.
Assisted in the conduct of 2017 Drug Symposium held at Boys Scouts of the
Philippines, Dagupan City Council Building on April 22, 2017.
Canvassed Training materials to be utilized for the upcoming Mass Training of
Grade 6 Teachers (MTOT) as per instruction by Madam Renata G. Rovillos,
Education Program Supervisor, LR.
Prepared an Inventory of Serials.
WE ARE ALL DRUG EDUCATION ADVOCATES, as everyone is re-oriented on the
countrys drug abuse problem, the ill effects of drug abuse and the legal
consequences of being involved in illegal drugs activities during the recently
concluded Drug Symposium held at Boys Scouts of the Philippines, Dagupan City
Council Building on April 22, 2017.

The Division Library has received thirty-eight (38) books and two (2) serials donated
by Komisyon sa Wikang Filipino (KWF), a government institution tasked to promote
and preserve Filipino language and other local languages on April 27, 2017.

A. ACTIVITIES
The Division Library has acquired two (2) serials (local newspapers) donated by Dr. Marissa
O. Perez, Education Program Supervisor, SGOD and Dr. Lourdes D. Servito, Assistant
Schools Division Superintendent on May 10 and May 19,2017 respectively. For the month of
May, the library has also received two (2) action researches and two (2) theses from private
individuals on May 22, 2017; and one (1) pamphlet (government publication) entitled
Dagupan City Facts and Figures 2016 from the City Government of Dagupan on May 24,
2017.

RELOCATION OF THE TEMPORARY DIVISION LIBRARY. Mr. Jan Vincent D.
Galvez, Librarian II and Ms. Clarita M. Tamayo, Supply Officer II led the ocular
inspection of the relocation of the temporary Division Library at the former
Boys Scout of the Philippines-Dagupan City Council Office, Division Training
Center.

A. ACTIVITIES
Provided assistance to all pupils, parents and teachers as the school opening
is approaching thru a Help Desks Oplan Balik Eskwela on June 1, 2017.
Followed up the memorandum on Division Seminar-Workshop on School
Library Operations and Management slated on June 30 to July 1, 2017.
Attended Division Office Mancom at Division Training Center on June 2,
2017.
Monitored first day of school opening to make sure that pupils were safe and
sound during the opening classes of Juan L. Siapno Elementary School.
Its school time again for all pupils as Mr. Jan Vincent D.
Galvez, Division Librarian does head count to Grade I pupils
of Juan L. Siapno Elementary School.
Attended a first Meeting by Division called and presided by the newly installed
Schools Division Superintendent, Dr. Lorna G. Bugayong, CESO VI on June
13, 2017 held at Teachers Hall.
SDS, Dr. Lorna G. Bugayong, CESO VI together with ASDS,
Dr, Lourdes D. Servito and CID Chief, Dr. Venus Maria SM.
Estonilo led the first Meeting to all CID personnel of SDO
Dagupan City.
Assisted in the conduct of Division Drug Education and Drug Testing held at
Division Training Center on June 16, 2017.
Prepared powerpoint presentations for the facilitation of the topics discussed
during the training.
Prepared Technical Assistance Report.
Assisted in the conduct of Orientation on Research Management Guidelines
held at Division Training Center on June 17, 2017.
Assisted in the documentation in the conduct of the Division Induction Program
of the Newly hired Teaching personnel held at Division Training Center on June
22, 2017.
Purchased training materials.
Prepared certificates and programs for the first Division Training for all
Teachers-Librarians.
Assisted in registration and documentation during DepEd Dagupan DRRM
Interagency Consultative Meeting held at Division Training Center on June 27,
2017.
Prepared Technical Assistance Plan for the month of July 2017.
Served as judge on Division Level Poster making and essay writing contest
held at Teachers Hall on June 28, 2017.
Dagupan City Councilor, Hon. Maybelyn D. Fernandez led
the awarding ceremony.
Monitored the second Simultaneous Earthquake Drill for this year at Juan P.
Guadiz Elementary School on June 29, 2017.
Prepared and monitored all the needed materials for the conduct of Division
Seminar-Workshop on School Library Operations and Management.
Official tarp is hanged and ready for the opening program
of the two-day seminar-workshop.
For the month of June, Division Library has acquired fifty-five (55) books,
fourteen (14) serials mostly souvenir magazines and six (6) unpublished materials
like thesis and compiled reports donated by Mdm. Froserfina J. Bravo, former
Schools Division Superintendent and now a Technical Consultant in Education at
City Government of Dagupan on June 13,2017.

B. TECHNICAL ASSSISTANCE
During the conduct of library visitation last November 22, 2016, Division Librarian
recommended to the School Head of Malued Elementary School to immediately relocate
its library in a safer place, since the former location occurred shaking at the 2 nd floor where
the library is previously located. Now, the library is relocated in a safer and bigger space and
currently organizing its collections during the validation of the said relocation on July 25,
2017.
C. BOOK DONATIONS
Division Library has received one (1) big balikbayan box which contained full of
childrens books from Bagong Kulturang Pinoy (BKP, Inc.), a registered non-profit
organization based in Massachusetts in US last July 27, 2017. The Turnover of book
donations to the Division Librarian was led by ASDS, Dr. Lourdes D. Servito. These
supplementary materials will surely benefit to learners as they will learn to love reading
and enhance their comprehension skills.

A. ACTIVITIES
Served as a program manager and one of the resource speakers on the first ever
Division Seminar-Workshop on School Library Operations and Management.
Grade 6 pupils were both actively and quietly engaged in the storytelling
session held at Mamalingling ES.
Cleaned the Room together with the Utility Worker intended for the Division
Library on July 17, 2017.
Mr. Danny Caguioa of SDO cleaned the floor of the Division Library located at
Division Training Center.
Mr. Danny Caguioa also thoroughly washed and brushed the tiles of lavatory of
the SDO library.
Attended the Orientation on the Establishment, Management and Operations
of the Library Hub on July 19, 2017 held at LRMDC-RO1, San Fernando City,
La Union.
Regional Librarian, Ms. Nids Cabal (standing) presided the orientation and the
first ever meeting of all SDO Librarians of Region 1.
Region 1-SDO Librarians took a group picture with Regional Key Officials led by Assistant
Director, Dr. Bettina Aquino.
Attended Kumustahan Meeting presided by Schools Division Superintendent,
Dr. Lorna G. Bugayong at Division Training Center on July 24, 2017.
Provided Technical Assistance to ten (10) School Libraries through its School
Librarians/Teachers-Librarians for the maintenance, enhancement and
improvement of school library services.

For the second time around, Division Library Division Library has received one (1) big
balikbayan box which contained full of childrens books from Bagong Kulturang Pinoy
(BKP, Inc.), a registered non-profit organization based in Massachusetts in US on July 27,
2017. The organization generously donated varied library resources composed of 334
books, 10 serials and 1 non-printed material to the Division Library of Dagupan City.
The Turnover of book donations to the Division Librarian was led by ASDS, Dr. Lourdes
D. Servito. These supplementary materials will surely benefit to learners as they will
learn to love reading and enhance their comprehension skills.
Assistant Schools Division Superintendent, Dr. Lourdes D. Servito happily
showcased the book donations composed of a quality reference and fiction
books from the BKP, Inc. on July 27, 2017.
SDO Librarian, Mr. Jan Vincent D. Galvez cheerfully received the book
allocation from the BKP.Inc. during the turn-over of the book donations led by
the Assistant Schools Division Superintendent, Dr. Lourdes D. Servito.
