AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, CLEOFE A. PANOPIO, resident of Brgy. San Agapito Isla Verde, Batangas City,
and GLORIA R. GARCIA resident of Sta. Clara, Batangas City, both of legal age, Filipino,
after having been duly sworn to in accordance with law hereby depose and state:

That we personally know FREDDIE ANONUEVO FAINA because we are close

friends;

That in his Philhealth records, his middle name was entered thereon as Rayos
instead of Anonuevo;

That his correct middle name may be confirmed from the copy of his Certificate
of Live Birth hereto attached as Annex 1;

That FREDDIE ANONUEVO FAINA appearing on the certificate of live birth and
FREDDIE RAYOS FAINA appearing on the PhilHealth records refers to one and the same
person;

That this statement was made not to deceive any legal authority or to evade any
criminal and/or civil liability;

That we are executing this affidavit to attest to the truth of all the foregoing
facts and circumstances and to inform the proper authorities that FREDDIE ANONUEVO
FAINA and FREDDIE RAYOS FAINA refers to one and the same person.
