CHAPTER 1 INTRODUCTION

This chapter covers brief descriptions such as:

1. Mining Business License (IUP) or Specific Mining Business License (IUPK) holder
status.
A. IUP or IUPK holder identity (name of the legal entity / cooperation / individual,
complete address, person in charge of the plan or activity)
B. Brief descriptions of license status (number, issued date, validity period, IUP or IUPK
of Foreign Capital Investment / Domestic Capital Investment status)
2. The total area of IUP or IUPK and supporting facility outside the IUP and IUPK area
(project area)
a. Total area of IUP and IUPK description which had planned for Manufacture
Operation Activities and supporting facilities;
b. Comprehensive description of supporting facilities outside the IUP or IUPK area
which used to support the activities of Manufacture Operation (project area).

3. Environmental Document Approval


A description of the Environmental Document Approval from an authorized agency
(number, date, name of agency).
4. Location and territory
a. A brief description of the Manufacture Operation location (villages, sub-districts,
districts, provinces, geographic positions) completed with the map of mining
location situation with minimum map accuracy at least of 1: 25,000 (one to twenty-
five thousand);
b. A brief description of transportation facility from and to the location of
Manufactures Operation activities.
5. Utilization of the land before and after the activities of Manufacture Operation stage
Description of land utility before and after Manufacture Operation activities.

CHAPTER II THE LAND CLEARANCE PLAN


This chapter covers land clearance plan for a period of 5 (five) years or in accordance with mine
age which includes:
1. Mining area
a. The description of the location and extent reserves area, methods of mining, life of mine,
the used equipment, location and area of land used for mining;
b. The description of the production plan, stripping ratio, and others.
2. Stockpile
a. The description of the location and area of land which used for:
1) Soil Landfill of the rooting zone;
2) Side and/or soil/ rock overburden landfill that cover the inside and outside of the
mine.
b. The description of the area of land and the location which used for the stockpile of mining
commodities;
c. The description of the location and area of land which used for supporting facilities waste
Stockpile / storage.
3. Roads
The description of the location and area of land clearance for mining road and/or transportation
road.
4. Sediment pond
The description of the location and area of land clearance for the manufacture of sediment
pond.
5. Supporting facilities
The description of the location and area of land clearing for the installation and facility of the
manufacture and refining process, offices, housing (camp or flying camp), workshops, and other
supporting facilities.

CHAPTER III RECLAMATION PROGRAMS


This chapter covers the Reclamation Program of the Manufacture Operation stage of the
impaired land within 5 (five) years or according to the age of the mine which annually specified
including:
1. Land to be reclaimed
Description of the Reclamation activities stages on the location and impaired land which will be
reclaimed including:
a. Former mining land;
b. Side rock and/or soil / rock overburden outside the mine;
c. Unused roads and/or transportation roads;
d. Former sediment ponds; and
e. Other supporting facilities.
2. Techniques and the equipment to be used in the Reclamation
The description of the techniques and equipment that used for land reclamation.
3. The stewardship of the Land
The description of the stewardship of the Land including the location and size as well as a
description of the type, the origin location of the material, and the filler material resources
volume (if conducted a back filling).
4. Revegetation
The description of plant species and number of plants, plant spacing, location, and size of land
that will be revegetated.
5. Civil works in accordance with the use of the Post-mining land
The description of the stewardship of the Land including location and area that should not be
revegetated (e.g. residential area, industrial area, tourism, etc.).
6. Utilization plan of the former mining pits (void)
A detailed description of the Reclamation plan on former mining land in the form of pits (void)
including:
a. Slope stabilization;
b. Security of the mined pits (void);
c. recovery and monitoring of water quality and water management in the mined pits (void) in
accordance with its designation; and
d. Maintenance of the mined pits (void).
7. Maintenance
The description of the reclaimed land maintenance, fertilization, pest eradication and plant
diseases, as well as efforts to maintain slope stability.

CHAPTER IV SUCCESSFUL CRITERIA


The description of the successful criteria of Reclamation covers the standard of successful of land
stewardship, revegetation, civil works, and final settlement.

CHAPTER V RECLAMATION COST PLAN


This chapter contains the cost plans required to reclaim the impaired land which specified annually for a
period of 5 (five) years or in accordance with the age of the mine. Reclamation cost calculation consists
of:
1. Direct cost
A description of the costs shall be calculated in the preparation of the reclamation cost plan
including:
a. Cost of stewardship of land, consisting of cost:
1) Surface arrangement - \ - land;
2) Dispersion of top soil;
3) Erosion control and water management.
b. Cost of revegetation, consisting of cost:
1) Soil quality Analysis;
2) fertilization;
3) Seed procurement;
4) Planting;
5) Plant maintenance.
c. The cost of prevention and acid mine water control;
d. The cost of civil works in accordance with the post-mining land allocation; and / or
e. Utilization of the mined pits (void) fee, consists of costs:
1) Slope stabilization;
2) Former mining pits securitization (void);
3) Recovery and monitoring the water quality and management of water in former
mining pits (voids) in accordance with its designation; and
4) Maintenance of mined pits (void).
2. Indirect costs
The description of the costs including the Reclamation calculation and if possible set by using the
reference standards as specified as follows:
a. Mobilization and demobilization costs of 2.5% (two point five percent) of direct cost or
based on the calculation;
b. Reclamation planning costs of 2% (two percent) to 10% (ten percent) of direct costs;
c. Administrative costs and third party profits as Reclamation operators of Manufacture
Operation Stages of 3% (three per cent) to 14% (fourteen per cent) of direct costs;
d. Supervision costs of 2% (two percent) to 7% (seven percent) of direct costs.
3. Total cost
The description of the total of direct costs and the indirect costs shall include the applicable
taxes and are made in Rupiah or US Dollar.

D. APPENDIX LIST
1. Map of land clearance plan situation with minimum map accuracy at least 1: 10,000 (one up
to ten thousand);
2. Map of the reclamation plan situation with minimum map accuracy of 1: 10,000 (one in ten
thousand).
Information:
If the area is very large and or consists of several mining blocks / production, which cannot be
described in 1 (one) map for each year, it can be described in several map sheets and equipped
with index map.
