I, JULIUS CECELIO TAEGA BRAZIL, of legal age, Filipino, and a

resident of Brgy 36, Sabang, Tacloban City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and state:

1. That I am the son of REMEGIO A. BRAZIL and MARIA NILDA R.

TAEGA, both residing at Brgy 36, Sabang, Tacloban City, Philippines;

2. That I was born on July 22, 1988;

3. That I am of legal age and have remained single all my life up to the
present;

4. That there is no legal impediment for me to enter into a contract of
