AFFIDAVIT OF SUPPORT AND CONSENT

We, FERGIENEL M. FRANGE and NERISSA O. FRANGE, both of legal ages,

Filipinos and residents of Block 15, Lot 3, Austery Subdivision, Mandurriao, Iloilo City,
after having been duly sworn to in accordance with law do hereby depose and state:

That we are the biological parents of FERGIE JOHN O. FRANGE who is presently
a Grade 11 student at the Philippine Science High School-Western Visayas Campus
with address at Bito-on, Jaro, Iloilo City. As parents, he is under our care and
custody and are responsible for his support as well as his education and other
needs;

That we hereby manifest and declare that we are giving our full consent for him
to join the 11th International Olympiad on Astronomy and Astrophysics to be held
in Phuket Thailand on November 12-21, 2017;

That for this purpose, we have sufficient and adequate financial capacity to support
and defray the said travel and other incidental expenses anent to the trip;

That we execute this affidavit to attest the veracity and truthfulness of the
foregoing facts and circumstance for the purpose of securing a clearance from the
Department of Social Welfare and Development and for whatever legal purpose it
may serve.
