PETITION
COMES NOW, the petitioner, through the undersigned counsel and to
this Honorable Office, most respectfully states that:

1. Petitioner is of legal age, Filipino citizen, widow and a resident of

Poblacion North, Solano, Nueva Vizcaya;

2. Petitioner is the registered owner of the certain parcel of land

covered by TCT No. T-152768 which is more particularly
described as follows:

A parcel of land Lot No. 6-B-1-B of the subdivision plan

Psd-(af)-02-027008, being a portion of Lot No.6-B-1 (LRC)
Psd-149663 situated in the Barrio of Poblacion,
Municipality of Solano, Province of Nueva Vizcaya, Island of
Luzon. Bounded on the SW., along line 1-2 by lot 6-A, Psd-
41252; on the NW., along line 2 -3 and on the NE., along
line 3-4 both by Lot 6-B-1-A, of the sub. plan; on the
SE.,along line 4-1 by Calle Luna 20.00m. wide. Beginning at
a point marked on 1 on the plan being S. 79 deg. 12E.,
281.99 m.from BLLM No. 2 Solano Cadastre . xxx
Containing an area of ONE HUNDRED TWENTY
SEVEN(127) SQUARE METERS more or less. xxxx.

3. On October 20, 2006 petitioner married Rodrigo Laciste in San

Juan, Metro Manila, a machine copy of their Marriage Certificate
is hereto attached as Annex A and made an integral part hereof;

4. Unfortunately, petitioners husband met his untimely death on

November 6, 2015, due to multi-organ dysfunction as evidenced
 by his Death Certificate, a machine copy of which is hereto
attached as Annex B and made an integral part hereof;

5. In order to straighten the civil status of petitioner and to avoid

confusion, there is an impending need to correct her civil status in
the said certificate of title particularly her civil status from
MARRIED to WIDOW

WHEREFORE, premises considered, it is most respectfully prayed

before this Honorable Office that the civil status of the petitioner in the
duplicate and office copies of TRANSFER CERTIFICATE OF TITLE NO.
T- 152768 be changed from MARRIED to WIDOW. Other reliefs just
and equitable are likewise prayed for.
