AFFIDAVIT

I, PHILIP CESAR D. MANZANO, Filipino citizen, of legal age, and resident of

De La Salle University, DBB-B, Dasmarias City, Cavite, after being duly sworn to in
accordance with law, depose and state: That - -

1. That I am the husband of ELIZABETH R. ARTISTA;

2. On May 1, 2011, we entered into a contract of marriage at Nueva Ecija;

3. Sometime in September 2016, we separated due to incompatibilities.

4. That her signature was required for the Voluntary Turn-Over form for the
Termination of Housing Loan Contract of the Home Development Mutual
Fund.

5. That since then up to present, I have no communication whatsoever from her

neither that I have any idea of her whereabouts;

6. Through this Affidavit, I hereby declared under oath that I and ELIZABETH
R. ARTISTA separated due to incompatibilities and since then up to present, I
have no communication whatsoever from her neither that I have any idea of
her whereabouts and for the same reason, her appearance and signature
cannot be acquired for the Voluntary Turn-Over Form.

7. I am executing this Affidavit to attest to the truth and veracity of the

foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 3rd day
of August 2017 in Dasmarinas, Cavite.
