15 Lot 23 Brgy. Emmanuel II, City of Dasmarinas, Cavite, after having
been duly sworn according to law, depose and state: That - -

1. That we have lived together as husband and wife since year

1997 and without any legal impediment to marry each other.

2. That we intend to enter into a contract of marriage with one

on other in order to legitimize our illicit union;
