AFFIDAVIT OF CONSENT

I, JAIME GOBOTIA, Filipino, of legal age, and resident of Purok 5,

Bautista, Dasmarias City, Cavite, after being duly sworn to in
accordance with law, depose and state: That - -

1. I am the father of a minor named, ELVIEVANE GOBOTIA who

was born on March 8, 2000 in Vinzon, Camarines Norte;

2. The said child will be travelling to Singapore for ASEAN SCHOOL

GAMES and will be accompanied by her coach FERNANDO DAGASDAS;

3. I hereby give my full consent and conformity to the said

travel of my above-mentioned child to the said country.

4. That I hereby guarantee that she will not be a public charge

to the government of Singapore;

5. I am executing this affidavit to attest to the truth and

veracity of the foregoing and for whatever legal purpose the same may
serve;
