JOINT-AFFIDAVIT OF DISCREPANCY

WE, MARIETA YONGIS, Filipino, of legal age and resident of Blk 4, Lot 27, Ville
De Soleil, Langkaan I, Dasmarias City, Cavite and CRISOSTOMO I. BUGAOAN, Filipino,
of legal age, and resident of #010D Sitio Bijia Brgy. Calibuyo, Tanza, Cavite, after being
duly sworn to in accordance with law, depose and state: That - -

1. We personally know WILFREDO MALIMBAN BENCITO as the son

of MANUEL BENCITO and RUPERTA MALIMBAN being our close family friend and
neighbor for a long period of time;

2. Unfortunately, when he secured a CERTIFICATE OF LIVE BIRTH,

he found out that his MOTHERS NAME is registered as VICENTA MALIMBAN instead
of RUPERTA MALIMBAN which is her mothers true name.

3. Moreover, his middle name was omitted in his Certificate of Live

Birth, it should be registered as MALIMBAN.

4. Through this affidavit we hereby declare under oath that his

mothers true and correct name is RUPERTA MALIMBAN and his middle name
is MALIMBAN.

5. We are executing this affidavit to attest to the truth and veracity of

the foregoing and for whatever legal purpose the same may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 27th day
of September 2016 in Dasmarias City, Cavite.
