JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, ROLANDO C. VELOSO, Filipino, of legal age and resident of 127

Aalley-F Tel. Compd., Brgy. 310 and DOMINADOR B. CAGNAYO SR., Filipino, of
legal age, and resident of Blk 31 Lot 44 46 Crossing Golden City, Salawag,
Dasmarias City, Cavite after being duly sworn to in accordance with law, depose
and state: That - -

1. We personally know JOSELITO S. NEPOMUCENO as the son of

JOSE C. NEPOMUCENO and CELERINA C. SANDOVAL being our close family
friends and neighbor for a long period of time;

2. We also know for fact that he was born on December 28, 1958 at
Paranaque;

3. That the cause of the discrepancies in his name was due to

misagreement of his parents whom gave the information, whereas JOSE
SANDOVAL NEPOMUCENO JR. was his fathers decision which is registered in
his Certificate of Live Birth; while JOSELITO S. NEPOMUCENO was his
mothers decision and which is registered in his Death Certificate.

4. Through this affidavit we hereby declare under oath that his true
and correct name is JOSELITO S. NEPOMUCENO.

5. Also know for a fact that the names JOSELITO S.

NEPOMUCENO and JOSE SANDOVAL NEPOMUCENO JR. refers to one and
the same person.

6. We are executing this affidavit to attest to the truth and veracity

of the foregoing and for whatever legal purpose the same may serve;
