I, MARITES T. DISIERTO , of legal age, Filipino, married and with residence

and postal address at Lupoc, Labason, Zamboanga del Norte, after having been duly
sworn to in accordance with law, do hereby depose and state that:

1. I am the Admin Assistant II assigned at Balas National High School located in

Barangay Balas, Labason, Zamboanga del Norte;
2. That the teachers and staffs of Balas National High School is using log book
and Daily Time Record (DTR) for the month of May, as the schools biometric
machine suffer technical problem as it did not function anymore;
3. That the entries I made in the log book and daily time record were true and
correct based on my personal knowledge;
4. If the new biometric machine will be installed in the Balas National High
School of Barangay Balas, Labason, Zamboanga del Norte, the undersigned
undertakes to comply and register for use of biometric machine when
reporting for work;
5. I voluntarily executed this affidavit to attest to the veracity of my foregoing
statements for the purpose of claiming my salary for the month of April,
2017.
