Below is a template for inputting and analyzing completed pre-/post-questionnaire. Please do not feel overwhelmed; you can
master the process in no time. The step-by-step directions below ensure an easy follow through. If you have questions about

Important note about this document: All templates for Student Surveys utilize the new program kit pre-/post-questionnaire. If
your pre-/post-questionnaire are different than those presented below, it is because the new tests are being added to the
program kits as inventory is used and materials are reprinted. To download the new pre-/post-test, visit
http://portal.jahub.org/servlet/portal/serve/20191. If a cell displays #DIV/0!, it is because data has not been entered in the
spreadsheet at this time.

This template was created for an evaluation design that includes Participating students only.

Attitudinal Questions (Post-Test Only)

Students answered a series of questions in response to
"Are these sentences true for you?"
1. Insert students responses to the attitudinal question
under the Post-Test Scores tab (Rows 23-30).

2. Insert as follows: 1=Yes, 0=No, 99=Don't know for

questions A through H for each student.
3. Use 1=yes and 0=no for the question "Would you
recommend this program to a friend?"
4. Lastly, type in student responses for the two things
they learned from the program.
Please note: Open-ended responses will not tabulate
automatically. For more help on how to summarize
student responses, please contact hspahn@ja.org.
