The undersigned Officer in Charge of Tagbilaran City Police

Station, after having been duly sworn to in accordance with law accuses
ELLENA MIAG-AO Y ADARNA, resident of Tamblot Street, Tagbilaran
City, Bohol, of the crime of Mutilation, committed as follows:

That on July 4, 2017 at 9:30 in the morning, in the Miag-ao

Residence located at Tamblot Street, Tagbilaran City, Bohol, Philippines, the
above-named accused with deliberate intent to mutilate, did then and there
wilfully, unlawfully and feloniously chop off the male sexual organ of the
victim, Divino Miag-ao, with the use of a Butchers Knife, 8 inches handle
to tip, while the victim was sleeping soundly in their bedroom. The victims
sexual organ, an essential organ of reproduction, was totally cut off to the
prejudice and damage of the victim Divino Miag-ao.

The act committed by the accused is a violation of Article 262 of

the Revised Penal Code of the Philippines. Contrary to law.
