PETITION

COMES NOW, Petitioner, for and by himself and unto this Honorable Office of the
Executive Judge, Regional Trial Court, most respectfully states:

1. That he is of legal age, single, Filipino and a resident of Bagumbayan, Tigbauan, Iloilo,
Philippines;

2. That he was born on December 9, 1988 at Guimbal, Iloilo;

3. That he is a member of the Philippine Bar having passed the Bar Examination given in
2016 and admitted in the practice of law in the Philippines on May 24, 2017 as evidenced
by his Certificate of Membership in the Philippine Bar, machine copy of which is hereto
attached as Annex A;

4. That presently, he holds office at Duremdes Nava Law Office with contact number (033)
320-3935 and email address navalaw@yahoo.com.ph;

5. That he continues to possess all the qualifications and none of the disqualifications to said
office;

6. Petitioner has paid his required membership dues for CY 2017 and Building Fund
contribution to the Integrated Bar of the Philippines (IBP) as per IBP Official Receipt No.
_______ dated _______, a photocopy of which is attached hereto as Annex B and
further evidenced by a copy of the Certification from the Integrated Bar of the Philippines
vouching for the same hereto attached as Annex C;

7. That he is a member of the Philippine Bar in good standing. With no administrative or

disbarment case filed against him, as evidenced by Certificates of Good Moral Character
 duly issued by the Office of the Bar Confidant of the Supreme Court, IBP Commission on
Bar Discipline, and Integrated Bar of the Philippines-Iloilo Chapter, copies of which are
hereto attached as Annex D, E and F, respectively;

8. That he has paid the Professional Tax of Three Hundred Pesos (300.00) for the year
2017, as evidenced by a Professional Tax Receipt No. ______________ dated
__________, machine copy of which is attached hereto as Annex G;

9. That also attached hereto and marked as Annex H, and forming an integral part of this
Petition another sheet of paper containing three specimens of his signature as well as his
recent pictures.

WHEREFORE, premises considered, it is most respectfully prayed to this Honorable Court

that the Petitioner be appointed as NOTARY PUBLIC for and within the territorial jurisdiction of
this Honorable Court.

Other relief consistent with law and equity are likewise being prayed for.
