As a parent/guardian, I understand that the CMS Musical needs the help and support of families in
order to make the show a success. Every family will be assigned committees and scheduled to provide
assistance at rehearsals and clean-up duties for shows. We ask that families plan to dedicate a
minimum 10 hours to support these activities. These hours will be spread out over the course of the
rehearsals and run from January through March.
Committees are broken into two groups, depending on when most of their volunteer hours are
needed. Please rank each group of committees below in preference order (1=I am most interested in
serving on this committee TO 7=I am least interested in serving on this committee). Please see the next
page for a brief description of each committee. We make every effort to assign you to 2 committees
that you prefer.
_____ I am a graphic designer and am willing to design multiple versions of the musical logo for
websites, posters, program, t-shirts, etc. (in EPS Vectored Art, JPG, GIF, PNG format and dimensions)
_____ I am willing to be a committee chair
_____ I am willing to be a committee co-chair
_____ I am willing to shadow a committee to be a committee chair or co-chair in the future
_____ I have business connections for possible CMS musical sponsorships
Please share any skills or connections that you are willing to share with the committees. (sewing,
construction, proofreading, business or sponsorship contacts, etc.)

We understand that the CMS Musical is a commitment both for my child and our family. We
understand that CMS Musical rehearsals are mandatory. Our family is committed to providing a
minimum of 20 hours of support to committees, rehearsals and show clean-ups to make this musical
successful.
