Introduction

To produce high quality products in an increasingly competitive marketplace,


the ability to efficiently discover, track, and correct incidents or failures
found during product development, testing, and operations is crucial. This

for tracking product failures and managing corrective actions. These can
range from so-called manual approaches using paper-based intake meth-
ods and handwritten forms, to a combination of electronic spreadsheets
and personal databases, to large enterprise systems that affect hundreds or
thousands of customers, suppliers, and engineers worldwide.

Nevertheless, the implementation of a cost-effective FRACAS that produces


an end result of more reliable products remains a complex and challeng-
ing undertaking. Decades worth of publicly available guidelines from the
United States Department of Defense (DoD) to NASA fail to address many
of the practical complications that exist. These complications are neither
process-related nor technical in nature, but stem from each companys or-
ganizational structure, goals, and expectations.
