AFFIDAVIT OF EXPLANATION

I, RIAN T. VASQUEZ, of legal age, Filipino, married and a resident of Brgy.

Mipangi, Nabunturan, Compostela Valley, Philippines, after having been duly sworn
to in accordance with law, hereby depose and say that:

1. I intend to pay for the penalties accrued for my Drivers License in

relation to my renewal of the said license before the Land Transportation
Office (LTO);

2. That on November 9, 2012, I was charged for committing the violation

of unregistered MV;

3. Some of the requirement of the said payment is presentation the

Official Receipt (OR) and Certificate of Registration (CR) of the motor
vehicle I was riding when I was charge by the said violation;

4. Efforts were exerted to locate said OR and CR, but in spite of

diligent search, it could not be found and the same are now beyond
recovery since it was lost due fire ;

5. I am executing this Affidavit to attest to the truth of the foregoing

facts in connection with the payment of penalties and my renewal of the
said license before the Land Transportation Office (LTO) and for whatever
legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 9th day of

September 2017 at Tagum City, Davao del Norte, Philippines.
