Safety, Risk and Reliability specific experience

Below summarizes all Process Safety related experience I was exposed to

either when running Projects at Sparrows, or lecturing at University to
students in Equipment design and process Simulation or as Process
Engineer in Discipline Engineering team or as Assistant Operation
Engineer:
TRAININGS - I have successfully attended the following trainings: Process Safety
Fundamental, Hazards and Operability Study (HAZOP), Layers of Protection Analysis (LOPA),
Hazard Identification & Task Risk Assessment (HITRA), Process Safety - BowTie and
Barriers, Inherently Safety Design, Management of Change (MoC), Hazard Area Classification.
HAZOP/SOA/LOPA I have supported many hazops, responsible for the Process Safety
Information collection and verification before the actual study, attended many sections as
observer and a full study as Scriber.
LNG PRODUCTION Operations I have spent 56 days acting in the field with the
Production operators, taking daily readings, understanding the processes flow, startup and
shut down operations, auditing maintenance and ongoing works.
RELIABILITY HEAT MAP I have undertaken responsibility to investigate and report the
status of all equipment across PSVM FPSO, collecting data from responsible parties, e.g,
Maintenance Supervisor and Operations Team Leader, and updating the Map with the
vulnerabilities, and all actions in place in support to the engineering team.
LEAKS AND OVERRIDES I did run a weekly monitoring of Leaks and Seeps across FPSO
and instrumentation overrides. All leaks were investigated to ensure actions are raised and
actioned identified for repair and operational constraints approved by the right entity. Use
engineering skills to estimate amount of gas released in a leak. Overrides were also
monitored for visibility, reason discussed weekly for opportunity to return related system
back to normal.
ORA and SORA I did manage Operational Risks systems by ensuring each ORA is correctly
categorized. Originate ORAs, participate in ORA Risk Assessments, risk assessing using both
8x8 and 6x6 matrixes. Ensure ORAs are sent for Approval within its validity. Facilitate
weekly meeting between onshore and offshore. Follow up all projects, actions and material
request on which ORA is pending to ensure proper closer. Post visible charts across FPSO to
ensure all on-board stuff are aware.
SAFETY AUDITS, PERMITS, HANDOVERS and ISOLATION - Through Self-verification, I did
check work execution in site and verified compliance with Permit for Work standards.
Undertook conversation with operators involved in the job, check if Toolbox has been held,
risk assessment has been properly made and if Permit covers all hazards and all barriers are
in-site during execution. Isolations were also audited to ensure they comply with the
standard. Check if an isolation has been properly applied, downstream valves safely opened
to check for any pressure or liquid build-up. Handover notes from shift to shift were also
checked to see if critical information was clearly communicated.
VALVE POSITION REGISTER - Manage Lock Open, Locked Closed valves in the Valve
Register, checking their current position as shown in the P&IDs against how it is in the field
and if it is properly tagged.
DOCUMENT CONTROL, PROCEDURE and DRAWING - Ensure Procedures used in site are
safe. Implement all proposed changes after an engineering check. Maintain a robust system
to ensure each mark-up made by technician in field, is revised, approved, written and issues
as new procedures revision.
MANAGEMENT OF CHANGE AND DEBOTTLENECKING STUDY - Attended engineering
study undertaken to increase PSVM production by 10 mbd over the initial design. Safety
study taken over all equipment design, operating parameters, valves proposed new opening,
for a safe operation. Identify plant change requirement, participate in MoC risk assessment,
engineering study and field changes.
PRODUCTION, PROCESS DESIGN AND ENGINEERING SIMULATION - Having built
extensive knowledge on hydrocarbons physical properties, phase behaviour and their
 application on design of processing equipment/plant, utilities, valves and process
philosophy. Mastering HYSYS simulations in University in support to disciplines such as
Thermodynamics, Fluid Mechanics, Equipment Design and Refining Technology taking
students and professors to their first experience in HYSYS.
FEED, PROJECT COST, TIME AND RESOURCES MANAGEMENT - Skilled in Projects in
developing cost, time and resource schedules in preparation of Proposal and tenders.
Controlling project costs, quality, risks and schedule. Creating documentation, meetings,
assigning resources as part of implementation of a Project. Organizing all expenses and
backups for any job final invoicing.
