Case :
One day in Happy Hospitals in Orchid rooms, there is patient with diagnose Diarrhea. Ms. ..........
,20 years old came to hospital at 08.00 am with diarrhea complaints already for 2 days. Patient
arrived in the rooms and she is accepted by nurse Reka.

Nurse 1 : Hello, good morning.

Patient :Good morning nurse.
Nurse 1 :How are you feeling right now?
Patient : Not very well I think.
Nurse 1 : Let me introduce myself, my name isReka,Im going to do your nursing
Admission this morning.
Patient : Hello Nurse Reka, nice to meet you.
Nurse 1 : Nice to meet you too. The first thing,Im going to check your details , is that
okay?
Patient : Yes,of course.
Nurse 1 : Can you tell me your full name?
Patient : My name is Ayu.
Nurse 1 : Whats your date of birth?
Patient : Its the first ofSeptember nineteen ninety seven.
Nurse 1 : Alright. What is your address?
Patient : Street Melati number 3, Jakarta.
Nurse 1 : Is your phone number still the same?
Patient : Yes, it is still the same.
Nurse 1 : Whats your occupation?
Patient : Student of college.
Nurse 1 : Thank you. Can you tell me why youre here today?
Patient : Sure. Im here because of diarrhea. I always defecate more than 4 5 times a
day.
Nurse 1 : That was so bad. Have you had any serious illnesses in the past?
Patient : Well, I never had any seriuos illnesses in the past.
Nurse 1 : Thats good. What about surgery? Have you ever had any operation before?
Patient : I never had any operation before.
Nurse 1 : Do you have any allergies to medications?
Patient : Yes I do. Im allergic to the shrimp. I have to be careful.
Nurse 1 : Yes you should be avoid eating shirmp. The last question now, can you tell
me the name of your responsible person.
Patient : Its my mother Maya. Ill give her number for you.
Nurse 1 : Okay, thank you. For the next, my friend will be here for you to asking some
quetion to you, is that okay?
Patient : Sure, Its Okay.
Nurse : Okay, Now I have to go.
Patient : Okay, thank you Nurse Reka.
Nurse : Youre welcome.

After that, nurse Desi coming to do observation.

Nurse 2 : Hello Ayu Good afternoon. My name is Nurse Desi.
Patient : Hello, Nurse Desi. Nice to meet you.
Nurse 2 : Nice to meet you too. Im going to asking you a few question, it is about your
medical story. So, you are here because of diarrhea right?
Patient : Thats right, I always defecate more than 4 5 times a day.
Nurse 2 : How are you feeling right now?
Patient : I am feeling so weak and loss of appetite.
Nurse 2 : I know what are you feeling. Did you remember, what did you do before you
got diarrhea?
Patient : Hmm.. before I got diarrhea, I had lunch at the canteen of my college.
Nurse 2 : And then? Did you remember what did you ate?
Patient : Hmm.. I think it was spicy fried rice, with shrimp.
Nurse 2 : With shrimp? But you have allergic with it.
Patient : I know, but I really want to ate that.
Nurse : Next time, dont do that dear. Because diarrhea can also be caused by
allergies of food.
Patient : Really? I got it. Thank you for your information Nurse Desi.
Nurse 2 : Youre welcome. Okay, Its enough from me, for the next my friend Nurse
Indah will be here for you.
Patient : Okay, thank you so much.
Nurse 2 : Youre welcome.

After that, Nurse Indah come to Patients room for doing physical examination.
Nurse 3 : Hello Ayu, Good afternoon. My name is Nurse Indah.
Patient : Hello, Nurse Indah. Nice to meet you.
Nurse 3 : Nice to meet you too, Im here for doing your physical examination. Is
that okay?
Patient : Sure, of course Nurse.
Nurses 3 : First, Im going to check your temperature. (checking) Your bodys
temperature is 37.8oC, you got fever. What are you feeling right know?
Patient : I am feeling uncomfortable and dizzy.
Nurse 3 : I will consultation with doctor about your medications. For now, I wil do
warm compress for you, the purpose is to stimulates the central thermostat to
lowerthe body's heat production. Is that okay?
Patient : Sure of course Nurse, its okay.
Nurse 3 : Allright, I will prepare for that. (prepare and doing warm compress).
Patient : Thank you Nurse Indah.
Nurse 3 : Youre welcome. Its enough from me, for the next my friend Nurse
Silvi will be here for you.
Patient : Okay, thank you.

After that, Nurse Silvi come to Patients room for doing the next physical examination.
Nurse 4 : Hello Ayu, Good afternoon. My name is Nurse Silvi.
Patient : Hello, Nurse Silvi. Nice to meet you.
Nurse 4 : Nice to meet you too, Im here for doing your next physical examination. Is
that okay?
Patient : Sure, of course Nurse Silvi.
Nurse 4 : First, Im going to check your blood pressure and pulse now. (checking) Your
blood pressure is 115/90 mmHg and your pulse is 80x/mnt everything is
good.
Patient : Thank you Nurse Silvi.
Nurse 4 : Okay. How about your defecate?
Patient : Still the same Nurse. This morning I have defecate for 3 times.
Nurse 4 : And then, what are you feeling right now?
Patient : I am feeling so weak.
Nurse 4 : Okay. For now Im going to check your weight (checking). Your weight is
54 kg.
Patient : Really? Before I got diarrhea my weight is 56 kg.
Nurse 4 : Okay, I will report this and consultation with doctor about your medications.
