CLINICAL PRECEPTOR EVALUATION OF STUDENT:

Motivation to Learn
Keenan was always very motivated to learn during this semester. He was very interested in the
different critical conditions of our patient population. He was continuously trying to connect
what he has learned in textbooks with the actual clinical picture. Keenan was eager to question
different treatment pathways, symptoms, nursing interventions etc. for patients with conditions
such as SAH (due aneurysms) complications such as vasospasms. Treatment of ICH, Stroke, and
many other neurological and general medical diagnoses that we encountered. Keenan became
very interested on the management of EVDs (external ventricular drains) and by end of our
preceptorship he was independently assessing the EVD, charting hourly on CSF output, ICPs,
and continuously keeping the drain level. This was a very impressive finding to see such
autonomy and confidence as many of our new nurse and even experience nurses feel challenged
by EVDs as their use is limited to trauma facilities such as TGH.

Patient Assessment
Keenan was very efficient with his assessments. By the end of the preceptorship he was charting
assessments on 2 patients and charting neuro checks hourly on both patients as well.

Communication
Keenan was great communicating with families and the health care team. some encouragement
was still needed to speak with physicians, specially during the major afternoon rounds.

Skills
Skills were performed outstanding. Keenan efficiently managed A-Lines, EVDs, Foleys, IV
lines. Efficiently inserted foleys, straight caths, IVs and assisted during the insertion of feeding
tubes.
Safety
Keenan always practiced with safety in mind. Any doubts he has he took to extra time to stop
and ask instead of just risking patients safety and his own safety. I personally was able to highly
trust him with the management of patients and administration of medications in his capacity. His
autonomy was outstanding.
Documentation
Keenan effectively documented all event and assessments. Vitals were charted correctly and
notes were properly charted on deviated values. Keenan would always question or inform me of
concerning findings and double check the correct way of documenting such findings.

Professionalism
Professionalism was probably Keenan highest quality. Not just with families or patients but the
rest of the team. Keenan was always proper on addressing me and other co-workers. His manners
and values were outstanding.
Overall, Keenan was an exemplar Nursing student. It was a wonderful experience that allowed
not only Keenen but also myself to grow in a personal and professional level.
