GUIDELINES FOR BLENDED SURVEYS

Standards for Blending Medical Rehabilitation Programs


into Surveys using another CARF Standards Manual as the Primary Manual

The following guidelines are provided for an organization seeking accreditation using CARFs 2017
Aging Services, Behavioral Health, Child and Youth Services, or Employment and Community Services Standards Manual
as its primary standards manual and that also wants to include Medical Rehabilitation programs in the same
survey for accreditation.
This is possible using what CARF terms a blended survey, which allows an organization to seek accreditation
through one survey for programs or services not contained in its primary manual. Standards relevant to
specific programs or services being added will be incorporated into a survey being conducted using the
primary standards manual in which those programs or services are not contained.
Note: Any organization that is unclear about which standards manual to use should contact CARF toll-free for assistance
at (888) 281-6531. It is important to contact CARF for clarification before starting the preparation process.
There are certain factors that CARF considers when blending programs and services, including:
The integrity of the programs and services.
Whether to incorporate standards from a related program or service section.
The primary standards manual that an organization uses. The primary standards manual (i.e., the one
into which other standards are blended) is determined to be the predominant focus and best match
overall for the services, programs, and settings for which your organization is seeking accreditation.
After reviewing these guidelines, be certain to check that:
You have the correct standards from the primary and additional standards manual(s); and
You have access to and/or completed the survey application identifying both the primary and additional
standards manual(s) on the Information for Scheduling tab and have included programs from both the
primary and additional standards manual(s) on the Programs for Survey tab.

Section 1. ASPIRE to Excellence
All standards in Sections 1.A. and 1.C.1.N. would be applied. Section 1.B. Governance is optional.

Section 2. The Rehabilitation or Service Process for the Persons Served
Standards from Section 2.A. through 2.E in the Medical Rehabilitation Standards Manual are applied
as identified in the following grid, based on the accreditation sought and populations served.
Applicable Standards grids at the beginning of each Section (2.A.2.E.) of the Medical Rehabilitation
Standards Manual provide additional guidance related to applicability of specific standards.

Section 3. Program Specific Standards
The following programs can be blended from the 2017 Medical Rehabilitation Standards Manual.
The program description and all applicable standards from the chosen section(s) would be applied
to the program(s) being surveyed.

Section 3.A. Comprehensive Integrated Inpatient Rehabilitation Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at beginning of Section 3.A. would be applied.
If optional specialty program accreditation is sought, the following would be applied based on the
specialty accreditation:

Pediatric Specialty Program accreditation must be sought in conjunction with at least one other program
in Sections 3.A.3.D., 3.L., or 3.O., with or without additional specialty program accreditation. The
program description and standards in the programs/services and any additional specialty programs to
which the Pediatric Specialty Program is linked would also be applied.

Section 3.G. Amputation Specialty Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at beginning of Section 3.G. would be applied.
Amputation Specialty Program accreditation must be sought in conjunction with at least one other
program in Sections 3.A.3.E. or 3.O. The program description and standards in the programs/services
to which the Amputation Specialty Program is linked would also be applied.

Section 3.H. Brain Injury Specialty Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at beginning of Section 3.H. would be applied.
Brain Injury Specialty Program accreditation must be sought in conjunction with at least one other
program in Sections 3.A.3.E. or 3.O. The program description and standards in the programs/services
to which the Brain Injury Specialty Program is linked would also be applied.

Section 3.I. Cancer Rehabilitation Specialty Program
The Program Description would be applied.
Standards as identified in the Applicable Standards grid at beginning of Section 3.I. would be applied
Cancer Rehabilitation Specialty Program accreditation must be sought in conjunction with at least one
other program in Sections 3.A.3.C. The Program description and standards in the programs/services
to which the Cancer Rehabilitation Specialty Program is linked would also be applied.

Section 3.J. Spinal Cord Specialty Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at the beginning of Section 3.J. would be
applied.
Spinal Cord Specialty Program accreditation must be sought in conjunction with at least one other
program in Sections 3.A.3.E. or 3.O. The program description and standards in the programs/services
to which the Spinal Cord Specialty Program is linked would also be applied.

Section 3.K. Stroke Specialty Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at beginning of Section 3.K. would be applied.
Stroke Specialty Program accreditation must be sought in conjunction with at least one other program
in Sections 3.A.3.E. or 3.O. The program description and standards in the programs/services to which
the Stroke Specialty Program is linked would also be applied.

Section 3.L. Interdisciplinary Pain Rehabilitation Program


The Program Description would be applied.
Standards as identified in the Applicable Standards grid at the beginning of Section 3.L. would be
applied.
If optional accreditation as a Pediatric Specialty Program is sought, program description and
standards from Section 3.F. Pediatric Specialty Program would also be applied.

Section 3.M. Occupational Rehabilitation Program


The Program Description would be applied.

Section 3.N. Independent Evaluation Services
