AFFIDAVIT

I, Ferdinand R. Villanueva Sr., Filipino citizen, married and of legal age,

and a resident of Block 11 Lot 11 Fiesta South Homes Salitran 2 City of
Dasmarias, Cavite, after having been duly sworn in accordance with law, hereby
depose and say:

1. That I am a Regular/Permanent employee of the Government of the

Republic of the Philippines, under the Department of Education, Division of
Dasmarias Cavite, Teacher III, of Paliparan 3 Senior High School;

2. That I am filing my Leave Of Absence from the period of 02 October 2017

to 01 December 2017, to give myself an ample time to prepare and to take
the BAR EXAMINATION which would commence on 05 November
2017.
