Officer 1 : Assalamualaikum, welcome to Surga Hospital. Im Dara and next to me is
Atika. Can we help you, Sir?

Mr. White Jhon : Yes Ms. Dara. Im White John and my wifes name is Adela White. Id like to
register my daughter. She is now in Emergency Room.

Officer 1 : OK, Sir. i have to fill this adminission form for the medical record. Alright,
whats your daughter name, Mam?

Mrs. Adela White : Her name is Monalisa White

Officer 1 : Had she ever been hospitalized her?

Mr. Jhon White : No, She Hasnt

Officer 1 : Where is the address?

Mrs. Adela Jhon : JL. Ayam Goreng no. 01 Lezat

Officer 1 : Alright, how old is she?

Mr. Jhon White : She is 17 years old

Officer 2 : is She a Student?

Mrs. Adela White : Yes, shes Senior High School student

lunch

examide by the doctor

Monalisa : Assalamualaikum, Dr. Dwira

Dr. Dwira : Waalaikumsalam, Monalisa. Do you have health problems?

Monalisa : Yes, Doctor. My skin feels itchy and there looks a lot of red spots

Dr. Dwira : When did you notice it?

Monalisa : It started after i have lunch

Dr. Dwira : Whats lunch menu do you eat?

Monalisa : Shrimp and spinach, thats all

Dr. Dwira : Alright, i will check your condition first

Monalisa : OK, Doctor!

skin because youve eaten shrimp

Monalisa : So, do i need a medicine from you?

Dr. Dwira : Yes, ill give you Antihistamine and you must drink it!
