writing: pp 24-25 (EL256)

The name of service user is Mr. Sompong Jaidee. His birthday is 21th
March 1969. He is xx years old. Mr. Sompong lives in a one bedroom
accommodation on the factory compound where he works. The
room is very small and dark with no windows. Moreover, he also
shares bathroom with 6 other factory workers. However, if he causes
a problem at work, he will lose his job and also his room.

Mr. Sompong has no permanent or temporary disability. In addition,
he has no significant medical condition or receiving any treatment.
His minor physical problem is body pain. His major physical problems
are lack of sleep and feeling fatigue. His sensory is perfect in sight,
comprehension, speech, and hearing. His memory and orientation is
normal while his mood is a server problem as sometimes he feels
confused and lucid.

His indicators of depression and anxiety are waking up early and
difficult to get back to sleep. He has poor appetite and weight loss.
He cant communicate well and feels withdrawn. He also has suicidal
thoughts and in risk of committing suicide.

Consequently, his behavior has been causing trouble at work with
both his superiors and his co-workers. He is at risk of losing his job.
As a result, he will lose his accommodation.

We recommend Mr. Sompong to visit his doctor and nurses at
nearby hospital more often. He should visit the hospital more often
than once a month. Though his work has taken much of this time, he
needs to spare some weekends to go to a group therapy. Medication
might be needed after couple visits and his symptoms havent not
been improved.
