Scaffold shelter
Scaffold shelters are erected on site using winding wire, this
practice is totally unsafe which may lead to collapse of shelter
anytime. Clamps need to replace winding wire to make shelters

scaffolding supervisor and will provide the avaialable clamps for
shelter on the sites

Driptrays
Driptrays are to be provided for all towerlights and for all the
3.2 chemicals containers used in the site. QBEC has to provide QBEC

Towerlight maintenance
Many towerlights in the site was observed with damaged

junction box, control panels, improper connections. It is required
to arrange proper maintenance of towerlights.
Electrical Inspection
There electrical powertools used in the site needs to inspected
3.4 quarterly , and a competent inspector is required for this job. QBEC
Electrical Engineer Shahul Hameed shall follow up and conduct
the quarterly inspection .
PPE
Workers are found to be working with no proper PPEs in the
3.5 site. There was availability issues regarding the PPEs before, QBEC
but now the PPEs are available and QBEC needs to take strict
action against workers who are reluctant to comply.
Fuel tank without cover
Fuel tank of the towerlight kept open without cover. This could
lead to serious fire in the site. Therefore its necessary to provide

cap for fuel tank , QBEC has to take action inorder to replace
the cover and to ensure that the cover is always with the
towerlight
Poor housekeeping
Housekeeping has to be ensured in sites at all times . Its
3.7 advised by Senior PM that there should be one hour allotted to QBEC
housekeeping each day.

Rebar caps
Rebars without caps has been observed widely in the site. The
3.8 survey team needs to take special care to provide rebar caps QBEC
while they erect rebars for level.
Rebar caps are now available in the store.
Toilet doors
QBEC
Damaged handles of toilets in the site was observed. This has
3.9 caused the personnel on site to use toilet without locking the
door. It is required rectify the door lock as soon as possible.

Sleeping on ground
Workers found Sleeping on the ground during lunch break at
QBEC site office, there is no adequate rest area at site office
3.10 and site as the number of workers are increasing day by day. QBEC
The rest area is being arranged in QBEC office area under the
supervision of Foreman Sharma.

3.11 Overflowing sewage QBEC

Overflowing sewage tank found in site due the mobile toilet

Contract Job System
Contract Job System has been observed in the site which
includes designating a work to a team of workers after fixing the
rate for the job and finishing it far earlier than the required time.
These kind of system of work leads to bypassing of safety rules
to cut short the time of work.
HSE has full authority to stop the work in case of any serious
3.12 unsafe act or condition. Apart from this all foremen and leadmen QBEC
shall sign a written agreement stating that they undertake the
complete responsibility of their workers .

All workers should follow safety rules, if not complying there

shall be deduction from salary. This shall be ensured by HSE
officers and Site Engineers.

Smoking Area
Designated smoking area separate for staff and workers is
required in QBEC office to avoid personnel smoking inside office
or in undesignated area.
3.13 QBEC
It is required that the personnels in the higher management
shows example to others by smoking only in the designated
smoking area.

New Workers/ Staff

New staff and workers has been observed in the site without
induction. It is advised that all personnels newly coming to the
siteare to be inducted and to undergo health surveillance before
starting the work or moving to site.

Engineer and Timekeeper needs to advise new workers and
staff to undergo induction prior to starting off the work.
Timekeeper needs to take responsibility to inform new comers
to gt inducted prior to start of work or moving into site.

Ground Condition
QBEC office area requires to be laid with milling materials to
tackle the clay type ground conditions during rain and control

the dust in office area.
The purchase of milling material is on process.

Inspection Report

Concrete Pump
Proper barrication needs to be ensured around the concrete

pump area to avoid unauthorized personnels and vehicles
moving into area nearby concrete pump.
Waste Management
Proper and timely removal of wastes from sites needs to be
ensured. Waste accumulating in the site is completely
4.2 unacceptable.
As per the procedure , the wastes from waste bin is removed
along with black plastic cover and these covers are collected by
boom truck. Timely Removal of waste must be ensured.
4.3 Banksman
The equipment operation shall be done along with banksman.

Overloading
Overloading of Wheel loader was found in the site. This could

lead to fall of materials and also injury to personnels from stones
falling from wheel loader bucket.
Scaffolding QA/QC NCR
Scaffolding materials needed for the close has been requested ,

as sson as the materials are available scaffolding supervisor
needs to close out the NCR.
Rest shelter Demo Completed
Rest shelter Demo was completed by Foreman Sharma , and

was approved consultants and hence need to fabricate more
number of shelters for all the sites.
Environmental Audit
Environmental audit is scheduled for 27 November 2017 , the
sites need to be prepared for the audit, with proper
5 housekeeping and showcasing proper waste management and
chemical handling.
The food waste bins shall be removed from sites as food is now
taken in mess hall
Break Timing
The break timing of QBEC is fixed to 11.30 am to 12.30 pm , All

the site personnels are to follow this. The buses will collect
personnels from site at 11.15 am.
