1. PURPOSE:
To establish guidelines for deployment of safe and effective work at height rescue operations.

2. SCOPE:
This procedure applies to all work at height rescue activities conducted at ( Al-Karrar Co.). KEC Work at height Rescue Team
will provide primary emergency rescue support for all permit required work at height conducted at ( DS02 , Stage 4).

3. Definitions

4. RESPONSIBILITIES

4.1 Managers
 Provide resources to conduct work at height rescue.
 Assure RT-WAH members are properly trained and equipped.
 inform contractor about any emergency case or incident
 support all workforce to complete the job in safe side
 provide proper rescue equipment

4.2 Supervisor
Upon arriving on the scene, the supervisor member will assess the following and advise all other personal
responding:
 What type of locationis this?
 Are there product storage hazards?
 Locate and secure the attendant or a reliable witness.
 Determine location and number of victims.
 Obtain blue prints, maps or have site personnel draw a sketch of the site.
 Determine the mechanisms of entrapment or nature of illness.
 Make a conscious decision as to this is a "rescue or recovery".
 Determine number of WAH points and locations.
 Determine electrical/mechanical/chemical hazards.
 Is responsible for the entire site emergency.
 Directs the entire emergency on scene.
 Is involved in the development of the site strategy.
 Is responsible for the tactical activities and decisions.
 Remains at the site
 Communicates with the RT-WAH
4.3 HSE Supervisor
 Ensure from PTW issued for job and RA completed
 Ensure all WAH staff are trained and certified
 Ensure that RT-WAH are trained and certified
 Ensure all documentation related WAH present and correct
 Ensure rescue equipment proper to job and checked before using and start work
 Ensure the MS-WAH understandable and clear for all RT- WAH also for all staff include the job
 Communicating with RT- team leader.
 Investigate any incident , accident or near-miss occur during job and reporting to management

4.4 Rescue Team Members


 Communicates with the supervisor .
 Performs actions as directed by the supervisor .
 Communication with HSE Supervisor

4.5 Medical:
 Assure all RT- WAH personnel receive appropriate medical examinations.
 Prepare all medic equipment at work location.
 Ensure that ambulance ready during work period.

5. PROCEDURE:

5.1 Notification:
Upon notification of a work at height rescue situation, supervisor will immediately dispatch the following via the
emergency channel of the plant radio system:
 Rescue team leader , (work at height rescue equipment cart) and all rescue personal
 Medical team leader and ambulance
 Call the local Fire fighter.
 HSE Team
 Management

5.2 Phase 1 Arrive On-Scene. Take Command.

5.2.1 THE PRIMARY ASSESSMENT


 Command willattempt to secure a responsible party or witness to the accident to determine exactly what
happened.
 An immediate assessment of the hazards present to rescuers must be done.
 Command willdetermine how many victims have been affected.
 A victim assessment willbe performed to determine how long the victims have been down, the mechanism
of injury, and the survivability profile of the victim.
 An early decision must be made as to whether the operation will be run in the rescue or recovery mode.
 Establish communications with the victim as soon as possible.
 Locate work at height permit and all other information about the space.

5.2.2 THE SECONDARY ASSESSMENT



 The Work at height
 Command willdetermine what type of work at height this is. This can be done by consulting the type of
WAH permit.
 What type of products are stored in this space.
 What known hazards are present; mechanical, electrical, etc.
 Location and number of victims affected.
 Diagram of work at height, including WAH and egress locations.
 Structural stability of the work at height.
 Hazardous material size-up.
 Review copy of work at height packet.

 On-Scene Personnel and Equipment
 Command will determine if there is an adequate number of properly trained personnel on scene to do the
rescue/recovery; a minimum of four properly trained personnel are required.
 Command will consider the effect of temperature extremes on personnel, and consider early rotation of
personnel operating on scene. For high humidity/high temperature conditions this rotation of personnel will
take place every 15 to 20 minutes. For moderate to low humidity/temperature conditions the rotation of
personnel will take place every 30 minutes.
 Command will consider the effects of rain on the hazard profile.
 Command will consider if the proper equipment is on-scene to complete the operation. This includes, but
is not limited to:
1. Atmospheric monitoring equipment
2. Intrinsically safe lighting.
3. Victim removal systems/equipment.

5.3 Phase II Pre-WAH Operations

5.3.1 MAKE THE GENERAL AREA SAFE


 Establish a perimeter. The size of the perimeter will be dictated by the atmospheric conditions, wind
direction, structural stability, etc.
 Stop all unnecessary traffic in the area.
 Assure that vehicles park near from incident if vehicles are running.

5.3.2 CONSIDER ASSIGNING THE FOLLOWING TEAM AS APPROPRIATE.


 Hazard Team
 Safety Team
 Rescue Team
 Medic Team

5.3.3 MAKE THE RESCUE AREA SAFE


 Command will assign a Hazard team to determine exactly what hazards and products are within the work
at height.
 Hazard Team shall do atmospheric testing in the location to determine flammability, and toxicity. Based on
readings, Hazard Team will advise Command of the proper level of personal protective equipment.
 Any product that is in or flowing in the work at height must be secured and blanked off if possible. It may
be determined that the location must be drained of any product prior to work .
 Any manufacturing or processing equipment must be shut down prior to work. If possible, all equipment
will be locked/tagged out and brought to a zero energy state.
 The structural stability of the work at height will be evaluated. If there is a potential for collapse,
appropriate measures must be taken to ensure the structural stability of the space.
5.4 Phase III (WAH) Operations. Victim Removal.

5.4.1 SELECTION OF PERSONNEL


 The proper personnel shall be selected to make the work at height.
 Command shall assign a Rescue team. Rescue team shall provide a minimum 2:1 ratio of personnel outside
the work at height to support personnel inside. This shall include a standby rescue team with a 1:1 ratio to
provide immediate assistance to personnel in the work at height.
 All in charge and back-up personnel will be properly trained in work at height rescue procedures annually
and capable of carrying out the rescue/recovery.

5.4.2 SELECTION OF PERSONAL PROTECTIVE EQUIPMENT


 Protective clothing shall be worn as required by the situation, depending on an evaluation of the hazards
and the products which may be inside the work at height. This may include helmet, gloves, proper footwear,
goggles and a stretcher.
 Rescue Team will determine the level of personal protective equipment by consultation with Safety team.

5.4.3 COMMUNICATION AND LIGHTING


 If the work at height has a flammable atmosphere, WAH personnel will have intrinsically safe or explosion
proof communication equipment. If this equipment is not available, Rescue Team may decide to use a (EX)
phone for communication or a message relay person. Remember, these atmospheres are considered
immediately dangerous to life and health (IDLH), until proven otherwise.
 If the WAH team is entering a dark work at height, Rescue team must ensure that the proper type of lighting
is used (EX).

5.4.4 ORIENTATION OF WORK AT HEIGHT


 If available and work into the work at height, the Rescue team, with the help of the responsible party, will
obtain diagram of the location. All WAH and backup personnel will be made aware of the layout of the
location to be entered.
 All in charge and back-up personnel, Command and Safety shall be made aware of the action plan and the
back-up plan work.

5.4.5 VICTIM REMOVAL EQUIPMENT


 Rescue team will consider MAN-LIFT equipment for victim retrieval from work at height.
 Rescue equipment will be certified and inspected.
 Rescue equipment will be checked daily before start job.

5.4.6 ASSESSING CONDITION OF VICTIM


 Upon reaching the victim, WAH personnel will do an immediate primary survey of the victim. If appropriate,
treatment will be started immediately.
 A quick but thorough secondary assessment of the victim will be done. If time permits, WAH personnel will
attempt to treat serious injuries prior to removal.
 If indicated, complete C-spine precautions will be administered. NOTE: Because of the difficulty removing
the victim from the location, optimum C-spine precautions may not be possible.
 If the victim is conscious, he/she will be encouraged to wear the appropriate breathing apparatus.

5.4.7 PATIENT PACKAGING


 After treatment of immediate life threatening injuries, the victim(s) will be properly packaged for removal
from the site.
 Prior to removal from the site, the WAH team will secure any loose webbing, buckles, straps, or any other
device that may hinder the extrication process.
5.4.8 VICTIM REMOVAL SYSTEM
 Prior to removal of victim, the WAH team will have stretcher to put the victim on it and fix the seat belts after
that 2 rescuer carry the victim to the MAN-LIFT, They accompany the victim on arrival at the point of
receiving treatment.
 As a general rule, WAH personnel will never allow the victim between the rescuer an the point of egress,
unless the situation dictates that one rescuer must pull the victim while the other rescuer pushes the victim
in order to achieve rescue.

5.4.9 TRANSFER TO TREATMENT


 Immediately after reaching the point of egress, WAH personnel shall transfer the victim to treatment
personnel.
 If the victim is contaminated from product inside the space, they shall be decontamination and corridor
shall be set up and used prior to transport of victim.

6. TRAINING:
 All rescue personnel are to be certified/properly trained to perform their assigned duties under the (KEC)
Work at height Program. It shall include:
 Communication.
 Emergency medical service training.
 Work at height program training.
 Incident command training safety Coordinators and Fire Inspectors.
 quarterly refresher training
