NOTICE OF DECISION
Dear Mr. Rubin,
In reference with our administrative meeting last August _____, 2017 at Tnalak Office,
Malaybalay City, Bukidnon. After carefully verifying the facts and circumstances we have duly
established.
 Your constant refusal to heed our notice of corrective action meeting last March 8, 2017
and March 25, 2017 to explain your absence without leave on; February 16, 20 & 25, 2017
and March 6, 2017 and leaving the work station without justifiable reason on January
26,27,30 and February 30, 2017 which caused prejudiced to our banana operations.
Moreover, the management painstakingly took steps to convince you to return to work but
it has fallen to deaf ears.
Based on the aforementioned act, you have wittingly, willfully and knowingly violated the
provision of our cooperative code of conduct the Offenses Subject for Corrective Action (OSCA)
on #I.B.2 to wit;
I. AGAINST WORK ETHICS and PERFORMANCE OF DUTY
B. Work Abandonment
2. Work Abandonment which causes cooperative loss or damage.
FOURTH OFFENSE: DISMISSAL

That after due consideration of the aforementioned facts, circumstances and grounds we have
decided that your services as employee of Tnalak Labor Service Cooperative is hereby
TERMINATED effective upon receipt of this letter.

Your final pay out and capital share, savings deposit and other disbursement if any shall be given
to you within 15 days working days from the receipt of this notice.
