Q1. Write a list of all types of records that are kept by a person in your job role or in the role for

which you are training. (In this case mental health worker)

Diagnoses and details of any health problems of the patient

Treatments that the patient might have received

How the treatments have helped the patient

Allergies of the patient to medications and other substances

Medication that the patient is taking currently

Bad effects that any form of medication have had to the patient

Results of health tests conducted in terms of blood pressure, blood tests, X-rays

Personal information of the patient such as age, address, work

Street drugs that the patient might have taken

Q2. How records might be kept:

Patient information needs be maintained in safe environment that will ensure the privacy and

safety of the data. This is achieved through proper maintenance of the records. One of the most

effective means of maintain such records is administrative maintenance where the people with

access to such information is the management and administration of the hospital. Tis will prevent

unauthorized access to patient information.

Q3. (a) Give an example of how a complexity issue may have associated factors or impacts. How

might this affect service delivery?

There is a need to understand that social determinants are crucial in improving the health

outcomes for mental health care organizations. These determinants play a huge role in

developing mental health and harmful substances use instances in the society. This means that

continued improvement of the mental health programs will provide support for the delivery of

Alcohol and other drugs (AOD) in the society.

(b) What might be some legal implications for mental health workers working with patients

affected by any of the following complexity issues?

Alcohol/drugs

According to the Code of ethics for Alcohol and other drugs (AOD), these patients should have

an access to the services they require. They need to have equal treatment as the other patients.

Mental health workers should be responsive to their needs.

Gambling

According to the Code of ethics for Gambling, mental health workers should be cost effective.

They are required to charge reasonable prices and use all the available to achieve the best

possible effect.

Domestic violence.

Mental health patient as a result of domestic violence are very frail. They are normally suicidal

and very disturbed. Thy need to receive proper care to avoid fatalities. Mental health workers are
4
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
required to deliver a positive outcome for the patients. The effectiveness is measured from the

patient’s perspective. They need to receive regular evaluation sessions.

Q4. Identify any systemic issues within an organisation using existing feedback from clients or

through gathering feedback and/or through discussing issues with colleagues. How would you

report these issues?

According to the feedback obtained from clients, it is clear that mental health care organizations

particularly the Australian Psychological Society have invested more of their resources on the

acute end of the mental health. Clients are concerned with the lack of investment in early

intervention and prevention. Clients are also concerned with why these organizations fail to

monitor and assess their spending on prevention techniques. Australian Psychological Society is

only concerned with providing treatment other than developing techniques to prevent the

occurrence of these conditions. As such, it is important to communicate these issues to the

relevant stakeholders. There needs to be the establishment of a new vision for the mental

healthcare organization. These reforms will be communicated through the participation in

promotion programs. The media needs to be involved in communicate the need for changing the

current approach adopted by these mental health organizations.

Q5. Research Activity (Focus on Australia)

Research/outline the correct procedure for dealing with the following complexity issues and

reference your sources:

a.Addiction

In Australia, addiction is a serious issue. People with any form of addiction are taken to

counselling where they obtain help that will assist them in controlling their addiction with the
5
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
anticipation of healing their addiction entirely. They are not taken to a mental facility but rather

taken to a counselling facility where they are taught how to control their addiction. In the

counselling facility, they are prevented from accessing the substances they are addicted to. They

are withdrawn from those facilities after regaining their normal lives and avoiding drugs. They

also need to prove that they are capable of living without those substances.

b.Domestic violence

This is dealt with by the law. There is no room for domestic violence in Australia. The law is

also made in such a manner that it incorporates all instances that might be regarded as domestic

violence to ensure people live in harmony in the society. The law has empowered law enforcers

to ensure that they have the capacity to handle domestic violence with the aim of reporting zero

cases of domestic violence.

c. child protection

Children are important members of the society. They are really protected in Australia. In

Australia, children need to undertake their education and are not required to work until they

attain the minimum legal age. The law protects children from all forms of abuse even from

their parents. Parents who are reported to abusing their children are stripped off their

parental duties and the children placed under foster care where they are able to access basic

need and receive love from their foster parents.

d. Chronic diseases

Chronic diseases are unavoidable realities facing all countries. However, Australia has been

credited with adopting one of the most effective techniques of dealing with chronic diseases in

the country. Australia has been able to fund and equip its health department. This enables the
6
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
department to undertake appropriate research and development to obtain cures and prevent the

chronic diseases from becoming national epidemics. Through research and development, the

country has been able to control instances of chronic diseases in the country by providing

appropriate medical attention.

e. justice system

In Australia, nobody is above the law. All citizens are liable for the crimes that they commit. It

does not matter your occupation or position in the government. The law must be adhered to. This

has been achieved by making the justice system very powerful in terms of investigating cases

and taking appropriate actions against the offender. The justice system is also a corruption free

zone which means that nobody is capable of getting out of the law through corruption. Tis has

been essential in improving the justice system and maintain law and order in Australia.

Q6. Choose a case from your current case load or use an anonymous case study. Identify risk

factors for social exclusion for that person. (In the context of mental health)

Social exclusion is the leading cause of stress and depression. People need social connection to

live normal lives by sharing their problems and interacting with people. Social exclusion leads to

lack of interaction which makes an individual a loner in that he/she is unable to share his worries

and happiness with other people. This leads to depression that causes mental illness.

Q7. Choose a policy listed on Page 10 of Student Workbook Three Section 3.3. Locate a copy

and give a brief outline of four (4) – six (6) items of its content. Explain how these items may

affect your own job role (or prospective job role) (in this case job role is mental health worker)

The National Mental Health Strategy is a commitment by the Australian government to improve

the lives of people with mental problem. The national government in conjunction with the local
7
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
government share the responsibility of providing mental health policies through the provision of

support services for mentally ill patients. These policies include:

 Reducing the impact and occurrence of mental disorders.

 Preventing the development of mental illnesses in all possible situations.

 Assurance of the rights and privileges of mentally ill patients.

 Reducing the impact of mental conditions in the society.

These policies are important to metal health workers. It will enable them to understand their roles

and responsibilities not only in the provision of mental healthcare services but also in the

reduction of occurrence. These policies also broaden the coverage of mental healthcare provision

by increasing their scope of coverage. Adhering to these policies will mean that mental

healthcare workers have to possess the essential documentations and credentials vital in service

delivery. This will ensure that they have the right educational background thus improving the

quality of mental healthcare.

Q8. How are options, directions and timelines agreed upon with patients in recovery-oriented care?

The involvement of patients in health care delivery is important in improving the overall quality

of medical care. Patients have their personal goals and preferences with regards to the medical

delivery. Medical practitioners need to negotiate with patients as it regards to the treatment options

available to the patient with regards to their medical conditions. This will create transparency

which is vital in improving the quality of medical care. Patients are very illiterate with regards to

medical terms and treatment in healthcare. As such, medical practitioners need to provide patients

with the proper directions in terms of drug use, diet, behavior and feeding habits. Doctors and
8
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
healthcare practitioners are the only teachers for the patients who are tasked with the responsibility

of providing them with proper directions with regards to treatment regimes. The directions include

the suitable timeline for taking the medication to ensure they heal completely and prevent

occurrence of the previous medical conditions. Failure to observe the required timelines is a recipe

for failed treatment which will lead to recurrence of the medical condition.

Q9. Create a template for a contingency plan, outlining all elements that should be included. Use

information below for Q10. NB I should be able to copy from word and paste into the pdf.

A contingency plan aims at organizing the response to an emergency situation. This contingency

plan highlights the steps to take on the event of an emergency of a person with mental health

problem. The plan aims at reducing fatalities and saving lives and preventing the condition from

escalating. Mental health is a serious issue that could lead to dangerous activities of the patient.

Mental patients need to be put in proper care to ensure they cause no harm to themselves and

people around them. The contents of the contingency plan include the contingency strategies and

the warning signs that indicates the patient is in trouble. On the case of health deterioration of a

mental health patient, the patient needs to be withheld to prevent him/her from incurring injuries

and damages since they are out of control. They need to be injected with a relaxing drug that will

cool them down. The goal of this is to ensure the patient is restricted from undertaking dangerous

activities. The history of the patient needs to be reviewed to determine whether it is a regular

problem and the most appropriate course of action based on past events. There are various signs

that will be visible from the patient due to the uncalled for behavior. The patient might exhibit

anger, distress, self-talk and even report an increase in risk-taking behavior. Aggression and

hostility are also warning signs of deteriorated mental condition. These signs might be as a result

of loss of employment, bereavement and end of a relationship. These factors need to be

9
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
considered as potential triggers to develop the most appropriate course of action to assist the

mental patient. The patient needs to receive proper attention and should not be left alone as they

are usually very suicidal.

Q10. Make a list of your own qualifications and work experiences and identify how these might

be a strength when handling patient cases.

Diploma in mental health, diploma in education, diploma in Agriculture. Work experiences: 7

years of teaching, 3 years of support work and 5 years as an Assistant nurse. Just answer the second

part of the question: identify how these might be a strength when handling patient cases.
10
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
Diploma in mental health is a crucial qualification which will allow me to handle metal patients

and offer them with proper care that will assist in their recovery. The diploma equipped me with

essential skills that will assist me in relating with the problems and issues facing mental health

patients. My five years as a nurse were vital in providing me with the relevant experience on how

to handle patients with care. During my time as a nurse assistant, I understood that nurses are

crucial in providing patients with hope and resilience to face their problems.

Q11. Which outcomes measures are used by an organisation? Explain to your Trainer/Assessor

how to complete/review outcomes measures, using any templates/paperwork, etc. available.

Organizations are formed to accomplish various goals and objectives. These objectives need to be

attained to ensure the organization is functioning and operating according to plan. However, the

firm need to undertake various techniques to determine whether it is attaining its intended

objectives. The most important technique to measure the outcome of the organization is through

the utilization of questionnaires which are issued to customers and other important stakeholders.

They are required to comment about the company and offer their feedback as to whether it is

achieving its intended objectives. Another technique for measuring the outcome of an organization

is through the number of clients of the firm. An organization which attains its goals and objectives

will definitely have more customers since satisfied customers become loyal customers.

Q12 Research at least three (3) local community services and/or supports and provide the

following information for each: (Focus on Australia, Queensland)

Neighborhood Watch Queensland

State Library of Queensland

YMCA Queensland Youth Parliament

11

Q13. Research and list which channels for knowledge exchange with local community
services/supports are available for an organization.
The Neighborhood Watch Queensland supports various groups in their efforts to connect and

strengthen communities to secure Neighborhood Watch Queensland

The State Library of Queensland assists in the location of resources of interest to people. It also

enables people to obtain items from the SLQ provided by the local library.

The YMCA Queensland Youth Parliament enables the youth to learn about politics, the country’s

political system and the government.

Q14. After reviewing feedback from service users, you find that many people have criticised the

lack of CALD resources in your service practice. How do you proceed? CALD stands for culturally

and linguistically diverse communities

Generalizing the needs and preferences of the people from different cultural groups needs to be

avoided at all costs. The customs of people vary based on their backgrounds and the region that

they grew in. Education also has a huge impact on people. Even though providing services to the

culturally diverse community might be challenging, it is vital to provide diverse services that are

accepted by all residents and users.

12
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
Q15. What is an organization’s procedure for gathering feedback from service users on the service

they received? Use information below

Organizations are required to obtain feedback from their service users in order to improve on their

service delivery and control on the erroneous areas. There are various techniques used by

organizations to collect feedbacks from their service users. The most effective technique is

preparation of questionnaires which are customized according to the information that the

organization aims at collecting. This is effective since it offers personal feedback that will enable

the organization to work on the areas largely mentioned in the questionnaires. Another technique

is through interviews. In this technique, the organization is required to interview various

stakeholders on the quality of the services provided by the company. This technique is crucial in

obtaining accurate information. However, it might limit the openness of customers as some are

afraid of offering negative feedback to the company.

Q16. Discuss the techniques you would use in the following scenarios.

a. A client is distracted, fidgeting in their seat and appears to lack concentration. What

could say to the client to address this, and note which technique this would be?

In this case, the patient seems to be bothered and very uncomfortable. The health worker needs

to ensure the patient is comfortable enough to share information. As such, the health worker

needs to become an active listener to ensure he understands the patient. This is achieved through

active listening and attending skills such as body language to make the patient comfortable.

Active listening will engage the patient engaged more thus develop a rapport tht will enable the

patient to share information.

13
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
b. Your client is talking very fast, bouncing from one topic or information sharing to

another. What techniques would you use to clarify that you are following along

correctly?

The patient seems to be confused which is a normal occurrence in mental health patients. To

ensure that the mental health worker is able to engage with the patient, the worker needs to

respect the patient to make him relaxed. This is followed by normalizing statements to

understand what the patient is communicating. Active listening will also enable the worker to

understand the statements made by the patient and come up with a conclusion based on the

information provided.

c. Discuss your attending skills in an interaction with the following client.

Your client appears to have come to their appointment smelling of alcohol, sad,

explaining that their best friend, a dog, has died. Which skills would be useful for

interactions with the client at this time.

The patient seems to be extremely dejected by the death of his best friend. The patient needs love

and compassion. The mental health worker should show empathy for the patient. The mental

health worker should become very approachable to ensure the patient opens up his problems as a

problem shared is half solved in cases of mental patients. Being approachable and listening will

enable the mental patient to trust and befriend the mental health worker thus assist in developing

rapport.

Q17. Give three (3) examples of the types of change that may be anticipated in the life of a service

user. Explain the possible risks associated with these changes and how you would manage them.

Use information below for Q19.

14
IMPLEMENT RECOVERY ORIENTATED APPROACHES TO COMPLEXITY
The life of a service user is expected to change as time goes by. These changes are associated

with technological improvement and ease of access of information. The anticipated changes

include:

Increased collaboration due to the presence of more diversity which combined with technological

innovations create an avenue for increased networking. This is important in gaining adequate

information from various stakeholders. However, this is disastrous as the service user will have

access to a lot of information that will provide him with options that might make him forget your

brand.
