LOCKOUT – TAGOUT PROGRAM PROCEDURE

FOR WYSONG 4’ SHEAR

A. MODEL # P100-129
B. SERIAL # P100-129
C. POWER SOURCE: VOLTS 220/440
D. POWER SOURCE: MECHANICAL & PNEUMATIC
E. CAPACITY: 50” x 3/16”

STEP 1 All Employees will be notified in advance by the person performing Lockout-Tagout of the
specific date, time, location & machine affected by the Lockout-Tagout.

STEP 2 EQUIPMENT ISOLATION: Locate all devices that control the flow of energy to the Wysong 4’
Shear (hereafter “Shear”).
a. Turn off at Control Panel: Deactivate the Shear by pushing the “STOP” button.
b. Turn off Power Control Breaker on the front of the Shear by turning the on/off switch to the
“OFF” position.
c. Switch circuit breaker marked “Panel # HV-1, Breaker # 17” located in the Breaker Panel on the
center West wall to the “OFF” position.

STEP 3 APPLICATION OF LOCKOUT-TAGOUT DEVICES:

a. Request Lockout-Tagout by Authorized Personnel.
b. Lockout-Tagout will be applied to Breaker Panel on circuit breaker(s) marked “Panel # HV-1,
Breaker # 17” on the center West wall.
c. Tagout will be applied to Power Control Breaker on the Shear.

STEP 4 CONTROL OF STORED ENERGY

a. Set the Shear to the down position so the blades cannot shift or settle.
b. Remove any air pressure off the unit by removing the quick release on the air line on the right of
Shear.

STEP 5 VERIFICATION OF ISOLATION

a. Make sure the power system has been isolated and de-energized. Press the “START” button to
see if the Shear will start. After verifying the system is isolated and de-energized, return to “OFF”
position.

STEP 6 Complete work to be done on the WYSONG 4’ SHEAR.

STEP 7 REMOVAL OF LOCKOUT-TAGOUT DEVICES:

a. Ensure all work is complete, remove all tools and clean up the area.
b. Ensure all personnel are clear of machine and that the Shear is safe to operate.
c. Request removal of Lockout-Tagout Devices by authorized personnel.
d. Switch circuit breaker(s) marked “Panel # HV-1, Breaker # 17” located in the Breaker Panel on the
center West wall to the “ON” position.
e. Turn on at Control Panel: Activate the Wysong 4’ Shear using proper power on procedure and
turning the on/off switch to the “ON” position.
f. Verify all parts are functioning properly, complete & sign any maintenance documentation and
turn in to the Shop Foreman.
