Surety Guidelines for the Execution of Scholarship Agreement

Upon receiving the scholarship award, you are required to fulfil the following terms as
part of the condition of acceptance:

1. The execution of a Pre-University Scholarship Agreement with YSD together with

two sureties satisfactory to YSD to guarantee your performance under the
Agreement.

2. The eligibility conditions of the sureties are as follows:

a. Relatives OR non-relatives with an individual income of not less than RM3,000
per month.

b. Sureties are required to attach certified copy of latest EA form, a letter from employer
certifying status of employment & current salary and a certified copy of identification
card.

c. Aged 45 years and below.

3. The failure of finding the right sureties may jeopardize the scholarship offer.

4. Please return this form during your Assessment & Development Centre session.
