AFFIDAVIT

I, Al P. Orlanes, of legal age, Filipino and a resident of 16 Medina St.

Masbate City after having been duly sworn to in accordance with law, hereby
depose and say:

That I am the proprietor of AL P. ORLANES ENTERPRISES located at

Medina St., Masbate City;

That Joana Z. Orlanes and Artimeo A. Balmes are no longer my

employees since January 2011 to date;

That they have voluntarily resigned from their jobs as evidenced by their
resignation letter dated January 3, 2011;

That here attached as Annexes “A” and “B” are copies of their resignation
letters ;

That I have executed this affidavit to attest to the truth of all the
foregoing and for whatever purpose this affidavit may serve.
