1.0 Purpose

This document establishes the instruction for the preparation and control of inspection and
test plan about building work and notification to the TPI for their selected inspection point in
a timely manner.

2.0 Scope

This document is applicable to the all works relevant to inspection and testing for building
work in KALSEL-1 2 x 100MW Coal Fired Power Plant such as :

- All kinds of building (Administration building, Water treatment building, etc.)

3.0 Definition

Inspection and Test Plan (ITP) : A document which identifies the main activities and test
to be performed during construction or installation.

Inspection : Conformity evaluation by observation and judgment accompanied as


appropriate by measurement, testing or gauging.

Test : Determination of one or more characteristics according to a procedure.

Review Points (R) : Report Review Point / Contractor / Subcontractor is required to


prepare a certificate or document attesting to the satisfactory performance of the related
test or inspection for review by the Employer

Spot Witness Points (SW) : Contractor / Subcontractor shall submit request for inspection
(Sampling, 10%) to the Employer decide whether they will attend such inspection or not.

Witness Points (W) : Contractor / Subcontractor shall submit request for inspection to the
Employer decide whether they will attend such inspection or not.

Hold Points (H) : Downstream construction and other activities shall not proceed without
attendance or any written approval of the Employer.

4.0 Responsibility

4.1 Site Manager or his representative responsible for

 Approval of ITP
 Overall supervision the inspection and test activities for Work.

4.2 Site QA/QC Manager or his representative responsible for

 Review of ITP including designation of witness and/or hold points.


 Controlling the supervision of inspection and test activities
 Assign the trained Quality Engineer to inspection and test activities

 Maintain and update the approved ITP and its master files

4.3 Quality Engineer is responsible for

 Preparation of ITP
 Selection of witness and/or hold points of site work to verify conformance to the
requirements of specified document (i.e. procedure, specification, drawings, code a
nd standard etc.)
 Performing the inspection and test pertaining to witness and/or hold points at the
appointed time
 Submitting the notification to the OWNER for their selected witness and/or hold po
int inspections of site work according to Inspection and Test Procedure.

4.4 Construction Manager or his representative is responsible for

 Preparing acknowledgement of ITP including designation of Witness/Hold points


 Overall supervision the work preparation for inspection and test activities
 Providing timely written inspection notification to Quality Engineer

4.5 Each section of construction (or Subcontractor) is responsible for

 Providing timely written notification (RFI issue) to Quality Engineer in accordance


with approved ITP
 Taking a prompt corrective action for any rejected items
