about how effectively you maintain and build workplace relationships.

Give this questionnaire to your team.

To obtain honest feedback is it recommended that the person filling


out the questionnaire is anonymous. The team should be informed
that the questionnaire is strictly confidential and the feedback is
anonymous. Remind the teams that the purpose of the questionnaire
is to assist you determine how effectively you maintain your
workplace relationships and areas where you need to make
improvements.
