ACTION PLAN

11.1. Problem identification

After the health survey, We identified health problems of Gende Kore kebele community using primary and secondary data sources.
For primary sources, we used an interviewer administered questionnaire, observation and interviewing of key informants where by
Secondary data was collected from health center and kebele health office.

Based on the assessment, the following health Problems were identified as listed in their hierarchy.

1. Latrine with no cover and hand washing facility

2. Low family planning coverage

3. Lack of appropriate solid waste disposal

4. Low per capita water consumption

5. No separate room for animals

6. increased prevalence of diarrheal diseases

7. presence of rodents and arthropods

8. Low TT coverage

9. Poor housing condition

10. children vaccine drop out
