REQUIREMENTS FOR TRANSFER OF DECEASED PERSON’S


STOCKHOLDINGS :

A. In case of Judicial Settlement:


1. Death Certificate certified true and correct by the Civil Registrar.
2. Certified true copy of Court Order.
3. Original copy of the Certificate Authorizing Registration (CAR) – if the applicable
taxes had been paid or a BIR Certification – if tax exempt.
4. Stock certificate/s registered in the name of the decedent for cancellation.
5. Specimen signature of transferee/s and photocopies of their valid IDs.
 For new holders residing within Metro Manila areas:
 Personal appearance to fill up specimen signature card and must present
valid IDs.
 For Individuals residing on Metro Manila addresses:
 Specimen signature card duly authenticated by the Broker.
 Photocopies of valid IDs.
 Bank certification from any bank certifying that they are depositors of that
bank and that the signature appearing on that certification is the same with
the bank’s records.
 For Corporate Accounts:
 Secretary’s Certificate indicating the authorized signatories with their
specimen signatures.
 Authenticated signature cards of the signatories.
 Articles and By-Laws of the Corporation.
6. Transfer fee – Php 100.00/certificate.
Cancellation fee – Php 20.00/certificate.
B. In case of Extra-Judicial Settlement:
1. Death Certificate certified true and correct by the Civil Registrar.
2. Original Copy or Certified True Copy of Extra-Judicial Settlement stamped
"RECEIVED" or "ANNOTATED" by the Register of Deeds.
3. Two-years Heirs Bond to be posted in favor of the heirs/creditors who may have been
unlawfully deprived of their shares/claim against the estate in the amount equivalent
to the market value of the shares for transfer.
4. Affidavit of Publication executed by the publisher of a newspaper of general
circulation in the principal office of Issuer Corporation stating that the required
Notice of Extra-Judicial Settlement or Affidavit of Adjudication had been published
once a week for three (3) consecutive weeks.
5. Original copy of the Certificate Authorizing Registration (CAR) reflecting that the
applicable taxes had been paid or if no estate tax is due, BIR Certificate of
Exemption.
6. Stock certificate/s registered in the name of the decedent for cancellation.
7. Specimen signature/s of transferee/s and photocopies of their valid IDs.
 For new holders residing within Metro Manila areas:

valid IDs.

NOTE: In compliance with Anti Money Laundering Act (AMLA) regulations, may we
request the heir/s to personally visit our office to fill up the specimen signature card/s and
present at least two (2) valid IDs.
