AFFIDAVIT OF UNDERTAKING

I, KEVIN LESTER A. MUTIA, of legal age, Filipino and a resident of De Oro,

Olingan, Dipolog City, Zamboanga del Norte after having been duly sworn in
accordance with law, hereby depose and state:

1. That I am the owner/proprietor of WIRES AND LINES INTERNET

CAFÉ;

2. That I am presently applying for Renewal of Business Permit for the

current year with City Government of Dipolog;

3. That I hereby undertake to comply in good faith the ordinances of

Dipolog City, such as the City Ordinance 11-226 or otherwise known
as, “Protection Hours for Minors” and City Ordinance 07-193 or
otherwise known as “Prohibiting Cyber Sex and Cyber-pornography”
and all other ordinances in relation to my business;

4. That this affidavit is executed in order to attest the truthfulness of the

foregoing narration of facts in compliance with the requirement of the
City Government of Dipolog as basis for the approval or application for
business permit renewal and for whatever legal intents and purposes it
may serve.

IN WITNESS WHEREOF, I am affixing my signature this 17th day of April,
