AFFIDAVIT
I, JOSELITO C. PICO of legal age, married, Filipino, with residence and
postal address at Brgy. Road, Pitogo, Municipality of Sta. Fe, Leyte, after having
been sworn to in accordance with law do hereby depose and state the following
narrations:

1. That I am the father of JOHN MICHAEL ESPINA who was born on May
31, 2002 at Alang-Alang, Leyte. The mother of the child is IRENE ESPINA.

2. That on May 3, 2003, my child – JOHN MICHAEL ESPINA was

baptized by Fr. Stephen Pesado in accordance with the Roman Catholic Rites at
Our Lord’s Transfiguration Parish, Palo, Leyte. The same was celebrated with
Rosalie Reazon, Felicidad Puevranas, Nelly Boy Navarro and Maricel Escanilla
as baptismal sponsors.

3. That at the time the child was born, Irene Espina and I, were not yet
legally married such that the child’s name which was recorded on his Baptismal
Certificate was JOHN MICHAEL ESPINA. The last name used was that of his
mother, IRENE ESPINA.

4. That on March 9, 2017, the child’s mother – IRENE ESPINA and I were
married before the Office of the Municipal Mayor of Sta. Fe, Leyte. Copy of our
Marriage Certificate is herein attached for ready reference.

5. That by virtue of the subsequent marriage between the mother of the

child and I, the child may now be legitimated and as such, can bear my family
name – PICO. Thus, I humbly request the office of Our Lord’s Transfiguration,
Palo, Leyte to make the necessary corrections/update of entry of the child’s last
name in his Certificate of Baptism from JOHN MICHAEL ESPINA to JOHN
MICHAEL ESPINA PICO.

6. That I am executing this affidavit to attest to the truth of the foregoing

facts and to support my request for correction and/or update of the last name of
my child JOHN MICHAEL ESPINA PICO. Likewise, this request is made in order
to conform his Certificate of Baptism to his other personal records.

IN WITNESS WHEREOF, I hereunto set my signature this February 1,

2018 at the Municipality of Palo, Leyte, Philippines.
