AFFIDAVIT OF OWNERSHIP

I, ARNULFO O. MORENO JR., of legal age, Filipino, married, a resident of Block 12, Lot
35, Cedar St., Grand Cypress, Brgy. Tungkong Mangga, City of San Jose Del Monte, Bulacan,
after having been duly sworn to in accordance with law, hereby depose and state:

1. That I am the lawful owner and possessor of subject motor vehicle of foregoing Motion
to Release, details of which are as follows:

2. I have acquired the said vehicle from its second owner, MR. SAMUEL FAJILAGOT,
of Kaligayahan, Novaliches, Quezon City, through direct purchase, for and in consideration of
the amount of One Hundred Forty-Five Pesos (P145,000.00), on September 24, 2013;

3. Aside from the delivery and transfer of possession of the vehicle, I have also received
from MR. FAJILAGOT, [1] the Original Certificate of Title No. 10920627, issued by the LTO-Manila
South District, dated October 28, 2010, [2] an Original Receipt of Registration, sometime in
January 2013, and [2] an open Deed of Sale of Motor Vehicle signed by the “Vendor,” the first
and registered owner, DR. NICANOR L. ENGRACIA, of 5101 San Antonio St., Makati City (copies
of which are attached for reference);

4. Since the Deed of Sale of Motor Vehicle originally signed by the owner, DR.
ENGRACIA, as Vendor, was still blank and not duly notarized, I was not able to file it at the LTO
office and request that the vehicle be transferred into my name. However, as required by law, I
filed for renewal of the vehicle’s registration every year thereafter (copy of latest Original Receipts
of registration’s renewal attached);

5. I am executing the foregoing Affidavit of Ownership to prove my lawful possession and

ownership of the subject vehicle, and in order to attest to the truth of the foregoing, as well as for
all legal intents and purposes it may serve.
