AFFIDAVIT

I, MARY WYNN C. NANONG, of legal age, Filipino, and with

residence at Brgy. Dapdap, Tayabas City, after having been duly
sworn to in accordance with law, hereby depose and states:

That I am the accountant of BERBETS OCTANE GAS AND

SERVICE STATION CO. with office address at Brgy. Libjo, Batangas
City;

That I have in my custody and safekeeping the Sales Invoices of

the said business;

That said invoices were in the storage room of the said business;

That sometime in May of 2017, upon checking documents

stored at the said room, I found out that several Sales Invoices were
damaged beyond repair by termites;

That sufficient efforts were made to recover the same but all
were in vain;

That upon checking with the accounting records of the

business, the damaged Sales Invoices are the following, to wit:

Sales Invoice Number 6463 dated March 5, 2014

Sales Invoice Number 6471 dated March 11, 2014
Sales Invoice Number 6477 dated March 18, 2014
Sales Invoice Number 6474 dated March 14, 2014
Sales Invoice Number 6482 dated March 21, 2014
Sales Invoice Number 6486 dated March 24, 2014
Sales Invoice Number 6491 dated March 28, 2014

That I am executing this affidavit to attest to the truth of all the

foregoing facts and to inform the proper authorities of the damage of
the said sales invoices;
 IN WITNESS WHEREOF, I have hereunto set my hand this 30th
