AFFIDAVIT OF CORRECTION

I, MAY LOVE VILLASORDA GALINATO, of legal age, Filipino, single and with
address at P-6 Paradise, Libertad, Butuan City, Agusan del Norte, after being duly sworn
according to law, depose and state:

1. That recently, in my printed Passport Application Form, I discovered that my name

appears as “MAY LOVE GALIANTO” instead of MAY LOVE VILLASORDA
GALINATO, and my place of birth appears as “Butuan City” instead of Gingoog
City;

2. That I am executing this affidavit to show and inform the Department of Foreign
Affairs (DFA) Butuan City, that my correct name is MAY LOVE VILLASORDA
GALINATO, and my correct birth place is GINGOOG CITY as shown in my
Birth Certificate attached herein;

3. That I attest to the truth of the foregoing facts.

FURTHER AFFIANT SAYETH NONE.
