3. Analyze the situations below; By using OSHA, provide the name of the incident, decide
whether this incident is a Recordable case or Non Recordable case; has lost work day or Non
Lost work day; state the number of Lost work day(s).

a. Jim was conducting repairs on a milling machine, which was not locked out. The machine
started, crushing his three fingers. The doctor asked him to undergo surgery and rest for
60 days.

b. Azli as a storekeeper was conducting checker activities at the unloading area. The driver
pulled the tractor trailer forward; causing him to fall down under the path of the trailer
wheels and sprains his ankle. Return to work on the following day, but was given an
administration work at the office for two weeks.

c. Four workers were hospitalized after performing maintenance work at one of the septic tank.
One of them was given a nebulizer to curb up his asthma and all were reported to work the
next day.

d. Raj was working at a grain bin. One part of the grain bin collapsed and hit Raj’s head.
Fortunately, he wore his hard hat. X-ray revealed no broken bones and was given first aid
treatment. However, later rashes on his skin.

e. Worker was felling trees in a tract with vines intertwined among the trees when one tree
uprooted another, which struck and killed the worker.

f. Worker receiving multiple bee stings while working outside an apartment building. Doctor
gave an anti-dote and return to work two days later.

g. Line leader was taking a sample from a line carrying hydrochloric acid. Suddenly, some of
the acid spilled onto his arm and his arm was burnt.

h. Worker gets food poisoning after ingested mushroom soup during annual company
dinner. Worker receives doctor’s treatment.

i. Workers suffers heat cramps from continuous exposure of hot process; admitted to
hospital for treatment; two weeks off.
