ASSIGNMENT

Introduction and background notes (vocational context)

As a learner, the core aim of this unit is to help you develop an understanding about your
responsibilities in ensuring and promoting the health and safety of people within the health
and social care workplace. When you complete this unit, you would have gained an
understanding of: (1) how health and safety legislation is implemented in the health and
social care workplace; (2) how health and safety requirements impact on care users and
providers and; (3) how to monitor and review of health and safety in the health and social
care workplace.

Scenario:
You are on a work placement at the Aldgate East Care Home as part of the requirements to
achieve your Level 5 HND in Health and Social Care. During your induction, you observed
some failings concerning the health and safety systems and procedures in place. For
example, you were not given a copy of the organisation’s health and safety policy and there
was no explanation about the responsibilities of employers and employee regarding safety.
In case of a fire incident, information about many important things were missing, including
fire exits, fire evacuation route and assembly points and any alternative routes. In addition,
there was no information about incident reporting procedures or the location of the nearest
first aid kit and contact details for first aiders. You are therefore of the opinion that the
systems, policies and procedures for communicating health and safety information was
inadequate and that stakeholders need to have a good understanding of their responsibilities
for managing health and safety, as specified in the legislation.

After three months of working at the Aldgate East Care Home, you observed that there are
problems in relation to the general requirements for risk assessment, including the
procedures for assessing manual handling. The case of a 90-year-old male resident that
died from injuries sustained in a fall from a hoist is a good example. An investigation by the
Health and Safety Executive, established that his death could have been prevented if a

better system for handling and moving residents, supported by appropriate staff training, was
in place. Furthermore, the sling was also not the one recommended by Care Quality
Commission as being suitable for someone with serious medical conditions. It is your
conclusion that the care home should have assess adequately and reduce all possible risks,
so far as is reasonably practicable. Another challenge was dealing with ethical dilemmas. In
the case of Mary, a 97 year old female resident, considering the principles of person-centred
care. She has been observed to be neglecting her hygiene while also refusing help with
washing because she considered such help intrusive. As a result, she developed a serious
rash on her inner thighs. When the condition worsened it was a dilemma to decide whether it
was necessary to wash her by force. A risk-benefit analysis was therefore imperative as she
is also considered physically and verbally abusive towards staff. There are implications for
the care home with regards to compliance to legal requirements. As specified by Health &
Safety at Work Act 1974 - all employers must ensure, so far as is reasonably practicable,
the health, safety and welfare at work of all their employees. In this case, the employer also
have a responsibility to keep service users and visitors safe. Failure to do so adequately at
the Aldgate East Care Home resulted in a criminal prosecution and employees and families
of service users suing for misconducts and negligent.

To prevent future occurrence, the Aldgate East Care Home now has a robust monitoring
and review system as part of the actions to promote health and safety culture. Rigorous
attention is now given to risk assessment and control, preventive information, training, and
incident data collection. As a result there is a 50% reduction in injury incidence rate over a
three year period.

Task 1A: Understand how health and safety legislation is implemented in the health
and social care workplace

What you must do - (Do a brochure) (Recommended up to 750 words)

Q1 In this section of the assignment you must produce a brochure to review the systems,
policies and procedures put in place to ensure effective communication of health and safety
information in the health and social care workplace in accordance with legislative
requirements. Use your own workplace experience to support your answers. According to
the case scenario, review how you will develop procedures for communicating health and
safety information. [P1.1, D1]

For example: Health and Safety (First-Aid) Regulations 1981 stipulates that it is the duty of
the employer to inform his employees of the arrangements made in connection with first-aid:
An employer shall inform his employees of the arrangements that have been made in
connection with the provision of first-aid. This includes the location of equipment, facilities
and key personnel involved in provision of first aid. Key indicators in keeping employees
informed include: displaying clearly and easily understood notices in prominent positions at
the site; catering for those with reading or language difficulties; inclusion of information
during induction training, ensure new employees are made aware of information etc.

Q2 Using a brochure format, in this section assess the responsibilities in a specific health
and social care workplace for the management of health and safety in relation to
organisational structures. For example for different stakeholders, (employer, employees, and
visitors, contractors, customers, partners etc.) as specified in legislation, in promoting
effective health and safety practices at workplace in the health and social care settings.

For example: Health and Safety at Work Act 1974 states that it is an employer's duty to
protect the health, safety and welfare of their employees and other people who might be
affected by their business. Employers must do whatever is reasonably practicable to achieve
this. Also, employees have a duty to take care of their own health and safety and that of
others who may be affected by their actions at work. In addition, workers must co-operate
with employers and co-workers to help everyone meet their legal requirements

 To achieve D1, conclusions have been arrived at through synthesis of ideas and have been
justified when reviewing systems, policies and procedures for communicating information on
health and safety in the health and social care workplace in accordance with legislative
requirements. You can use your relevant health and social care workplace experience to support
your answer.

Task 1B: LO1 Understand how health and safety legislation is implemented in the
health and social care workplace

What you must do - (Do a brochure)

Q3 For this task, you should analyse health and safety priorities that are appropriate for a
specific health and social care workplace setting. That is, examine specific health and safety
areas that require serious attention to safeguard the health and well-being of service user.
Linking your analysis to the case scenario, analyse areas of health and safety that you will
consider as priorities - manual handling, risk assessment, RIDDOR, COSHH, safety aids
(e.g. walking aid, wheelchair, hoist); security systems (e.g. door locks, cameras, gates,
alarms, patrol; maintenance e.g. (ventilation, temperature control, buildings) etc. You can
also link your answer to examples drawn from your own health and social care workplace
experience. A relevant case study from the Health and Safety Executive website
(http://www.hse.gov.uk) can also be used to support your analysis. [P1.3, M1]

 To achieve M1, an effective approach to study and research has been applied when analysing
health and safety priorities appropriate for a specific health and social care setting or relating your
answer to your own health and social care work placement experience. You can use a case
study to support your answer.

To produce a brochure:
The whole TASK 1 requires students to produce a brochure which answers the assessment
criteria. Below are steps for designing a brochure on Microsoft Word
1) Open a Word document.
2) On the upper left, click the "File" menu.
3) In the left vertical column, click the "New" tab.
4) Select "Brochures and booklets" from the available templates.
5) Select "Brochures" from the available templates.
6) Choose the kind of brochure you wish to use.
7) Include some pictures where appropriate

customers and the work of practitioners in the health and social care workplace

Effective assessment is the key to care planning that produces positive results (QCS, 2015).
A care plan is an agreement between a service user and a health professional to help the
service users manage their health day to day. It can be a written document or something
recorded in your patient notes. Knowing that you have good understandings of care planning
and risk assessment, in preparation for the admission of new residents, your organisation
has given you the responsibility to carry out suitable risk assessments which details the real
risks present within the care setting.

Q4 In this section, you should analyse how information from risk assessments informs care
planning for individuals and organisational decision-making. Consider identifying and
analysing key principles of care planning and risk assessment. Then do a hypothetical risk
assessment, using an appropriate template (you can find a template online) [P2.1, M2]

Q5 You should analyse the impact of one aspect of any health and safety policy. Link
analysis to your own health and social care work experience. [P2.2]

Q6 You should discuss by giving relevant example(s) of how dilemma(s) encountered in


implementing health and safety policies can be addressed. Here you can give an example of
a case study but you must also discuss what the dilemma in the case study is. [P2.3]

Q7 Finally, you should analyse the effects of non-compliance with health and safety
legislation in the health and social care workplace. [P2.4, M3]

 To achieve M2, relevant theories and techniques have been applied when analysing how
information from risk assessments informs care planning for individuals and organisational
decisions making about policies and procedures

 To achieve M3, logical principles have been applied and justified when analysing the effect of
non-compliance with health and safety legislation in a health and safety workplace

Task 3: LO3 Understand the monitoring and review of health and safety in the health
and social care workplace

What you must do (Role Play and Factsheet) (Recommended up to 750 words)

Measuring the health and safety activities of a care home is an important way of identifying
where improvements are required and what actions to prioritise. Hence, the Management of
Health and Safety at Work Regulations 1999 require employers to put in place arrangements
to control health and safety risks. That is, assessments of the risks to employees,
contractors, customers, partners, and any other people who could be affected by an
organisation’s activities. Organisations are also required to record significant findings in
writing (if there are five or more employees). Thus, it is imperative to have arrangements for
the effective planning, organisation, control, monitoring and review of the preventive and
protective measures that come from risk assessment.

Given this context, your manager has asked you to coordinate a systemic monitoring and
review of health and safety in your care home. You will need to gather information from staff
and evidence of good practice that can be presented to inspectors from the Health and
Safety Executive.

needs of individuals at the centre of your practice. Consider the issues of compliance;
training; practices; and interactions with individuals, groups and agencies. In this section you
may want to use Gibbs reflective cycle in reflecting on your own performance in helping to
meet the health and safety needs of the service users. [P3.3, D3]

The purpose of a role play is to encourage students to think critically and to develop their
initiatives in providing practical solutions to health and social care issues. Students will
improvise as they simulate a relevant scenario in the class environment to demonstrate how
you would

 promote a positive health and safe culture
 evaluate your own contributions

1. Students will be divided into groups of 3 to perform the role play


2. Then, individuals will produce a factsheet based on their group’s role play and this
would be submitted in the final submission of the assignment
The purpose of a factsheet is to give readers precise information without wasting their time.
Too much information than what is needed might be a barrier to effective communication of
health and social care messages.

 One or two pages is appropriate


 Make it very clear what you want communicate - Bold, text boxes, and graphics add
emphasis
 Remember to include references from quality academic sources. In electronic
communications you can offer links
 Make it readable - use at least 12 point font
 Keep the text brief – It is difficult to read tons of information in a small font
 Keep the most important information in the first paragraph - what the issue is, what
action is needed, and label the main message(s)
 The fact sheet must be self-contained - do not refer to previous documents
 Use bullets points when appropriate
 Give clear information, - do not only list " audit of risks” instead explain the points
appropriately.

 To achieve D2, substantial investigations have been planned, managed and organised when
analysing the effectiveness of health and safety policies and practices in your health and social
care workplace in promoting a positive health and safe culture. Here you can use your actual
workplace practice to include into your answer.
 To achieve D3, self-evaluation has taken place to evaluate your own contributions to placing the
health and safety needs of individuals at the centre of your practice. In this section you may want
to use Gibbs reflective cycle on reflecting on your own performance in placing the health and
safety needs of the service users in your practice.

3. Preparation guidelines of the Coursework Document


a. All coursework must be word-processed.

b. Document margins must not be more than 2.54 cm (1 inch) or less than 1.9cm (3/4 inch).
c. Font size must be within the range of 10 point to 14 point including the headings and body
text (preferred font size is 11).
d. Standard and commonly used type face such as Arial should be used.
e. All figures, graphs and tables must be numbered.
f. Material taken from external sources must be properly referenced and cited within the text
using Harvard standard
g. Do not use Wikipedia as a reference.

4. Plagiarism and Collusion


Any act of plagiarism or collusion will be seriously dealt with according to the College
regulations. In this context the definition and scope of plagiarism and collusion are presented
below:

Plagiarism is presenting somebody else’s work as your own. It includes copying information
directly from the Web or books without referencing the material; submitting joint coursework
as an individual effort.

Collusion is copying another student’s coursework; stealing coursework from another student
and submitting it as your own work.

Suspected plagiarism or collusion will be investigated and if found to have occurred will be
dealt with according to the college procedure. (For details on Plagiarism & Collusion
please see the student hand book)

5. Submission
a. Initial submission of coursework to the tutors is compulsory in each unit of the course.
b. Student must check their assignments on ICON VLE with plagiarism software Turnitin to
make sure the similarity index for their assignment stays within the College approved level. A
student can check the similarity index of their assignment three times in the Draft
Assignment submission point located in the home page of the ICON VLE.
c. All Final coursework must be submitted to the Final submission point into the unit (not to
the Tutor). A student would be allowed to submit only once and that is the final submission.
d. Any computer files generated such as program code (software), graphic files that form part of
the coursework must be submitted as an attachment to the assignment with all
documentation.
e. Any portfolio for a unit must be submitted online.
f. The student must attach a tutor’s comment in between the cover page and the answer in the
case of Resubmission.

6. Good practice
a. Make backup of your work in different media (hard disk, memory stick etc.) to avoid distress
for loss or damage of your original copy.

7. Extension and Late Submission


a. If you need an extension for a valid reason, you must request one using an Exceptional
Extenuating Circumstances (EEC) form available from the College examination office and
ICON VLE. Please note that the tutors do not have the authority to extend the coursework
deadlines and therefore do not ask them to award a coursework extension. The completed
form must be accompanied by evidence such as a medical certificate in the event of you
being sick.
b. Late submission will be accepted and marked according to the college procedure. It is noted
that late submission may not be graded for Merit and Distinction.
c. All Late coursework must be submitted to the late submission point into the unit (not to the
Tutor). A student would be allowed to submit only once and that is the final submission.

9. Glossary

1. Analyse: When you analyse you break an issue into its constituent parts. Look in
depth at each part using supporting arguments and evidence for and against as well
as how these interrelate to one another.
2. Assess: When you assess you weigh up to what extent something is true. You
persuade the reader of your argument by citing relevant research but also remember
to point out any flaws and counter arguments as well. You conclude by stating clearly
how far you are in agreement with the original proposition.
3. Critically evaluate/analyse: Give your verdict as to what extent a statement or
findings within a piece of research are true, or to what extent you agree with them.
Provide evidence taken from a wide range of sources that both agree
with and contradict an argument. You come to a final conclusion, basing your decision
on what you judge to be the most important factors and justify how you have made
your choice.
4. Explain: When you explain you clarify a topic by giving a detailed account as to how
and why it occurs, or what is meant by the use of this term in a particular context. Your
writing should have clarity so that complex procedures or sequences of events can be
understood; defining key terms where appropriate, and is substantiated with relevant
research.
5. Review: When you review, you look thoroughly into a subject. This should be a critical
assessment and not merely descriptive.
6. Discuss: When you discuss, this is a written debate where you are using your skill at
reasoning, backed up by carefully selected evidence to make a case for and against
an argument, or point out the advantages and disadvantages of a given context.
