Assignment # 4: Case Study on Rehabilitative Services (20 marks, 10%)

Mr. Smith is a 60 year old with prostate cancer that had metastasized to his spine. He recently underwent spinal decompression in the lumbar
region. He has been in the hospital for 2 weeks following the surgery and has very little movement and strength in his legs. His goal is to return
home to live with his wife. He is currently unable to dress, toilet, transfer or walk independently. He would like to be as independent as possible
with these, but according to the doctors, will not likely be able to return to independent unassisted walking.

1) Identify the steps of the rehabilitation process in this case (6 points).


The first step in the rehab process is referral, this is usually done by a doctor, CCAC, or he could do a self referral. After he is referred an
assessment must be done, by law, this portion must be completed by a registered therapist. The assessment will allow the therapist to
move to the next step which is creating and a plan, the intervention plan will be carried out by the OTA or PTA and should focus on
reaching his goals. The intervention plan is next, this is the main area for the OTA and PTA, this could be a dressing program,
strengthening exercises, and gait aid training. An evaluation will be done by the therapist to see if the plan is working and that progress
is being made, if there is none they would go back assessing the patient and modifying the plan to help him succeed. If the evaluation
goes well than the patient may be discharged.

2) Describe the specific role of the OTA/PTA within this case (1 point).
The intervention plan- gait aid training, transfers, dressing program, ect.
The intervention plan is the core role for the OTA/PTA, the assistant will be doing most of the exercises and activities with the client,
while the OT or PT will not see the client as often, only to address concerns and reassess them. The intervention plan as stated before
could include dressing programs, exercises, modalities programs, and patient education, just to name a few.

3) Develop a specific short term goal for Mr. Smith (1 point).


Mr. Smith will be able to perform dressing program independently in 3 weeks.
4) Develop a specific long term goal for Mr. Smith (1 point).
Mr. Smith will be able to ambulate with gait aid device 30 ft by week 8.
5) List the three types of consent that Mr. Smith can provide (3 points).
Oral consent, implied consent, and written consent.
6) Identify three health care services which will be utilized in this case (i.e. spinal decompression surgery in the hospital). Explain how
each service will be funded (i.e. OHIP pays surgeon for the procedure). (6 points)
1) Long term care in the hospital: paid for by OHIP
2) X-ray scanning: paid by OHIP
3) Physiotherapy: in hospital is paid by OHIP, once discharged will be paid out of pocket, or by health insurance

7) Describe a challenge in the health care system which might affect Mr. Smith (2 points).

There are many challenges within the health care system that may have an affect on Mr. Smith, one challenge he might face that there is more
demand then supply when it comes to health care. For Mr. Smith this might mean that he may not receive adaquent care once released from
the hospital. If he required at home care it may be insufficient, in-consistent, and poor quality due to the huge demand on our health care
system.
