Dear Doctor,

Re: Mrs Betty Olsen

I am writing to refer Mrs Olsen, an 83 –year –old widowed who is a resident of our centre. She needs
urgently admission to hospital due to persistent chest pain.

With regard to her past medical history, Mrs. Olsen has been managing for hypothyroidism,
hypertension, glaucoma and arthritis since 1997 until now .She has been receiving some medication
such as, karvea, oroxin, timoptol eye drop and norminson and also golden glow glucosamine.

On 09/07/09 Mrs Olsen has received a Mylanta for indigestion and took panadol for shoulder pain.
She could not sleep well .Today’s morning she looked very tired and anxious with blood pressure
180/95 mmhg and was bedridden. Her doctor notified that said she will be visited on 11/07 09.

Today’s evening her condition became worse .she refused dinner and has nauseous, shoulder and
back pain. Her blood pressure was 175/95 mmhg and took norminson 1 tablet and 2 panadol at 10
pm .I checked her again at 10/45 pm, she is distressed, pale, sweaty and her blood pressure is
190/100 mmhg .I called to ambulance immediately.

Please note, she has allergic to penicillin. Mrs Olsen has a son and his phone number is

I would appreciate it if you could admitted her to your hospital for further management .Should you
require any further information regarding Mrs Olsen please do not hesitate to contact me .
