The School Psychologist

Dear Sir/Madam,

Re: Aiden Cooper

I am writing to refer Aiden,a 12-year-old boy who is a year 6 student that is reluctant to have activity
at school and has problem to learn such as reading and writing .He has no friend and is not sociable
at school.

He lives with his mother, who is full time housewife and his father is working as busy accountant.
Aiden has an older brother and a younger sister.

With regard to his medical history, he is suffering from mild asthma and has no remarkable illnesses.
Although his body mass Index is 24, he is small for his age.

His school records show that he has a history of regular absences during 6 years. He frequently has
visited the school health centre with complaining of headache, stomachache, nausea and dizziness
.In addition he would not like to join in activities in the class unless persuade him and recently he
received extra study at home .His mother notified but seems is not concerne.Aiden feels anxiety and
is sensitive when talking about his visits to the health centre and is concerned that other children are
talking about him.

I would appreciate it if you could arrange him to be visited by the school psychologist regarding
possible psychological problem and also encourage his parents to investigate his health by their
general practitioner. Should you require any further information please do not hesitate to contact
me.
