REQUIREMENTS BEFORE PRESENTATION/DEFENSE:

1. Grade Form with proper class codes for LEC AND LAB (2 COPIES: 1
FOR UGRC, 1 FOR PROMOTER)
2. ACCOMPLISHED Peer Evaluation Form (2 COPIES: 1 FOR UGRC, 1 FOR
PROMOTER)
3. PRINT 3 COPIES OF THE RUBRIC FOR ORAL PRESENTATION, ONE FOR EACH
EVALUATOR
4. PRINT 3 COPIES OF THE RUBRIC FOR RESEARCH MANUSCRIPT, ONE FOR
EACH EVALUATOR
5. PRINT 3 COPIES OF THE RESEARCH MANUSCRIPT, ONE FOR EACH EVALUATOR
AFTER THE PRESENTATION:
1. Encode the corrections/recommendations of your panel during the
oral defense

2. Print one copy of the entire manuscript (APA FORMAT - Arial, Font
12, double space and margin of 1.5” on the left side 1” on all
other sides) with the prescribed contents and have it checked by
YOUR RESPECTIVE PROMOTERS NOT LATER THAN May 8,2017

3. After evaluation, THE PROMOTER SHOULD SIGN the approval sheet

4. Have the manuscript hard bound with the prescribed color (navy
blue and silver letterings) and headings.

5. Pass the :
a) hardbound copy together (with its soft copy saved in a
CD with label)
b) 2 print copies of the journal (soft bound)- please use
white cover. The journal should be accompanied by an
author’s information page that contains names of the
authors, their institutional affiliations and contact
information. This serves as cover page and is not included
in its pagination.

c) 2 CD’s- should be attached /taped inside the back cover of

the journal. Label the case with the title and name of
authors. The files should include: whole journal, a
separate file for title and abstract only(follow the format
for the abstract)
d) ACCOMPLISHED RUBRIC FOR ORAL PRESENTATION
e) ACCOMPLISHED RUBRIC FOR RESEARCH MANUSCRIPT
f) GRADE FORM ACCOMPLISHED AND SIGNED BY THE PROMOTER
