A. Description of the Learner:
Patient Z, a 24 years old woman, single, Roman Catholic, born on June 3, 1992. She is currently residing at Aurora Hill, Baguio City. She can read and
write. She knows how to speak Filipino and English. G1P0 Pregnancy Uterine 39 weeks Age of Gestation. Her chief complaint was a sharp, shooting and
throbbing pain brought about by labor rated as 10/10. “araaaaay koooo! Sobrang sakit dito sa may baba!” as verbalized by the client. Uncomfortable as
manifested by irritability with persistent sweating and grimaces.

Subjective Cues:
 Name of client: Patient Z
 24 years old
 Single
 Claims that she can read and write
 Claims that she can speak Filipino, English and Ilokano
 Chief Complaint: Pain rated as 10/10 characterized as sharp, shooting and throbbing pain
 “araaaaay! Sobrang sakit dito sa may baba”

Objective Cues:
 Female
 Presence of Grimaces and persistent sweating
 Uncomfortable and Irritable

A. Learning Need: To know the possible non-pharmacologic methods for reducing the pain.

B. Learning Style: : The client can easily remember and learn new things when listening (during the absence of pain)

C. Learning Diagnosis: Knowledge Deficit: Nonpharmacological methods for Pain as manifested by client rated pain as 9/10 with loss of control.

D. Goal: The client will be able to correctly choose and apply one type of nonpharmacological methods to conform her status.
